It's when you are injecting two QA insulin doses close together before the first one has flattened out, so say you injected today at 1pm and you injected again at 3.30pm you would still have IOB (insulin on board) so this is referred to as insulin stacking as most insulins have a 3-5 hour profile. Hope that makes sense
