Hi, Me again, Well seen Doc and can't be prescribed Metformin, as my Kidney function is only 60% so I am being prescribed gliclazides. Can you diabetic boffins explain this to me, what it means, what the advantages and disadvantages are, and what the implications of my Kidney function are.

kidneys can improve with good blood glucose control, so i would look at it again in 6 months and your Dr would have referred you on if you had CKD complications

Stage 2: Slightly decreased GFR between 60‐89 mL/min/1.73m2
If your kidney function is at stage 1 or 2, you only have CKD if you have albuminuria, haematuria, a pathological abnormality or a structural abnormality.

gliclazide works by increasing the output of the pancreas.

For me, the more carbs we eat the more carbs we want. They don’t give up easy and it’s biochemical

Hi. Can you let us know your BMI. If you have excess weight and are therefore insulin resistant, then Gliclazide may be adding insulin when you already have too much but can't use it. If you are near normal weight the Glic may well help. It sounds like a low-carb diet is key here? What sort of diet do you currently have?

I think my BMI is around 30 not sure, I have got excess weight, I have a fairly healthy diet, I don't drink alcohol hardly at all. Weakness is cake but I haven't had much of that of late, but I have been piling on the weight, feeling worse all the time. If you are correct what should I have been prescribed. My diet hasn't been Low Carb but I am going to give that a whirl, but confused as I was always told because of Kidney problem I shouldn't have a high protein diet. I have also got hypothyroidism.

Help I can't cope with feeling worse than I am already.

you aren't going to have a high protein diet, it's normal protein ..low carb and fats replace some/all of the lost carb energy
you need a HCP to go through it with you, if you lived in southport, I know who I would suggest.

You can let us know your BMI. When you are excess weight, you need to make yourself near normal weight. The Glic will well help. It sounds like the key is the diet. Go. I want to know what diet do you currently have?

At present I eat what previously I would have considered to be a healthy diet, but having done a lot of reading over the last few days I can see that it is carb loaded, I also ate a lot of fruit, I suppose a typical day would be,
Yoghurt for breakfast with coffee
Sandwich for lunch
Pasta rice or Pots with either meat or fish, casserole chilli bakes etc
Just lately I have had a craving for ice lols
Don't drink alcohol
Poss once a month out for coffee and a nice cake
Must admit through out week, a few cakes or flap jack as a treat.
That's what I mean carb loaded.

I haven't until the last couple of weeks got much exercise, I have been too shattered, but whilst I have been off work I have been walking about 3 times a week for about 40 mins. I am now going to increase this to daily and from Monday really going to vastly reduce the carbs. Can any of you advise where I should go to get a book or recipie please. I have an I pad so if you know of an electronic source that would be good. Off work for another 2 weeks and really want to get myself into a routine for when I am back at work.

@daisy1 has a useful basic guide to help you manage your diabetes and will post soon.
Take it slowly, this is a marathon not a sprint and it will take a while to get your head around how to manage things.

The main tools you will need is a meter and test strips and a food diary to see how foods affect you as an individual. People can tell you what they do but it does not necessarily follow that it will work for you so you need your meter as your guide.
Ask all the questions you need to and someone will answer.

Hi. Thanks for the feedback. Ref meds, Metformin is often prescribed if overweight but it is not ideal with kidney problems which may be why you haven't been given it. Yes, some of those carbs could be taken out which should help with the weight. Watch out for sugar in yogurts (I have no sugar-added yogurts). Many cakes and similar are loaded with added sugar so read the labels and go for the occasional plain cake. Alcohol is OK in sensible quantities.
