Anorexia recovery and high CBG's

Hi, I was wondering if I could receive advice as to whether or not this is worth discussing with my GP and whether anyone else has been in a similar situation. :)

For my whole life my BG has been within normal ranges, a bit on the low side (slightly below 4) in the mornings. My fasting and random BG were monitored among other things regularly by my GP due to my bouts of anorexia, so I have a very good idea as to what my normal is. I had also received finger-prick tests after eating meals or high sugar foods/drinks, and they always came back below 5.5. My eating disorder never seemed to have any significant effect on my BG.

In January this year I had another relapse into anorexia and was hospitalised in April with refeeding syndrome. Once admitted, I was given a single 5% IV dextrose solution, and after 2 hrs my BG was 18. After 4-5hrs it decreased to 15.4, then 11.4. I also tested high for ketones and had acidic blood pH. The doctors called the diabetes specialist (as there is a close family history of T1 & T2 diabetes) and he said that because I hadn't been eating properly, my pancreas had most likely temporarily suppressed its amount of insulin secretion, which is what caused my BG to spike after receiving a small amount of glucose.

I was told by the gastro team and diabetes specialist that with NG feeding and following a structured meal plan by the dietician that my BG should quickly normalise to healthy levels. I was kept in hospital for 1.5 months for weight restoration, stabilise biochemistry and CBG's. They were doing daily blood tests to monitory my magnesium, potassium, phosphate, renal function and FBC, and finger-prick tests every 3 hrs. For the first few weeks my BG kept falling into mild hypos (somewhere between 2-3) if I missed a single meal, and I would have hypos upon waking in the morning. I was accused of not following my meal plan due to the hypos, when I was truly eating. After that, my BG settled into normal levels, my fasting glucose ranged from 4-5 and I was happy.

Since being discharged from hospital, I have no nutritional deficiencies, am a healthy weight (BMI 20), have adequate body fat (21%) and have been adhering strictly to my meal plan, which consists of eating at regular times a low GI diet. Initially I was gaining weight and energy, but then over the last 3 weeks I have been feeling very tired and fatigued and unable to put on any weight. My Mum suggested I buy a glucose meter to test myself in the mornings to see if I was having any more hypos. What has surprised me, is that my fasting glucose ranges from 5.8- 6.8. Sometimes after meals I need to go for a massive wee, so I decided to test myself a few hrs after eating. E.g. after eating chicken curry with a small portion of rice, my BG was 11.7. Another day, after having an apricot danish, it was 9. Today I had a handful of salted crisps, and it was 8.4.

I have found the only way to keep my BG normal post-meals is to eat very small portions of low GI carbs and pack my plate with lean protein and non-starchy vegetables, but I am not a fan of doing this as my dietician said I need to have a sufficient amount of carbs in my diet and avoid losing weight. I have tried looking up advice online and all I can find is info suggesting weight loss, something which I cannot do, as I have been told if I lose weight again I'll be hospitalised. I am at a bit of a loss as to what to do, unsure if this is a normal part of anorexia recovery that'll settle in its own time, or if it's worth talking to my GP about. If anyone has any experience or advice related to this, I would be most grateful. x

It's always worth talking with your GP. It's great you have a meter. Keep a record of your blood levels this always helps a doc work out what's happening..

I would ask your doctor for a c-peptide and GAD antibody test just to rule out LADA.

Hi there,

As I am not a health professional I can't comment on what might he happening with your blood glucose levels. But what I do know is that certainly, your anorexia may have affected your pancreas and insulin secretion/sensitivity and that this may settle down with time. I would definitely stress that you should concentrate on your recovery, for now, keep your weight stable and stick to your food plan. Perhaps go back tot your doctors in a month or two and ask for a full blood test to see where you are at.

In the meantime feel free to use the diabetes and eating disorder forum for extra support if you feel it might be of help.
