I'm 65 type 2 diabetic since 1996 I take novalin r for my fast acting insulin and novalin n for my long lasting insulin. . My problem the doc I had three years ago ordered me an insulin pump ( Medtronic's). its been in my cub bored for 2 years, Because of ins, changes I have had to change endocrinologists twice and both Doctors have not put me on the pump .I don't know why and they tell me that the pump is not reliable. I have severe neuropathy and have had heart bypass surgery I have a stent in my left leg and an artificial vein in my right leg.. My sugar is still out of control. I get the feeling like they are treating me like a lost cause. . What can I do to get help from a reliable diabetic Doctor. I live in Ormond Beach Florida. USA. There are little docs to choose from. in this field . Most are booked full., Any help will be appreciated Thank You Steve

first thing is to get your BG in range. the pump can come when it comes. when you find a dr or clinic that will work with you.

what is normally recommended here is the miss a meal basal testing first
may need to convert back to USA numbers
http://www.diabetes-support.org.uk/info/?page_id=120

A workbook with USA numbers and math rules, it has a good troubleshoot at the end and basal adjustment is on page 34

http://www.bd.com/us/diabetes/download/insulin_adjustment_workbook_complete.pdf

it will be easier if you are on a low carbohydrate diet and it will also help you lose any excess weight. that also makes the diabetes harder to manage
http://www.dietdoctor.com/lchf it’s a long page and a video

http://lowcarbdiets.about.com/od/lowcarbliving/a/Food-Cravings.htm because carbs don’t give up easy.

I'm T2 and don't take insulin....take this advice with you when you seek medical advice to help you
novolin n is an intermediate insulin that you take twice a day, normally about 12 hr apart
did you forget your morning dose of novolin n ? or as per the basal testing you would raise the dose by 10% and redo the test in 2 days

specific dosing advice
http://reference.medscape.com/drug/humulin-n-novolin-n-insulin-nph-999006

the T1 will be back online in the UK morning

This may mean that your basal (long acting) dose is too low. Take as many readings as you can and get them to your doc! They should figure out what is going on.

Hi Horvys, welcome. Hopefully someone will be able to give you more info on your own management program, but as a T2 I agree with Jack's advice about low carb diets which you say you are trying. Best of luck and let's see if we can help you turn some of this around.

If you start trying to change the carbs you are eating without understanding the action of the insulins you are taking then you may find that you have problems.

I don't use a low carb regime but if you do want to try it can I suggest that you get some advice from someone who has some knowledge of how to reduce insulin gradually in your circumstances.
There is a Scottish Dr who has had quite a lot of experience with lower carb diets.(she used to be on this forum, and I didn't always agree with her philosophy but that is besides the point; her advice for low carbing was usually pragmatic and based on experience and not what I consider extremist)
She has together with a T1 diabetic , recently released an ebook which discusses various levels of lower carb diets , with varied targets and carb amounts according to peoples situations (ie very different targets and strategies for a 14 year old girl with raging hormones to a young fit man and again different targets for an older person with complications) She has a great deal of advice about how to use various types of insulin whilst reducing carbs gradually to more moderate levels . There is also so advice on how other non diabetic medications need to be taken into account.

Yeah, a gradual transition is a good idea. I immediately went low carb, and I still maintain it was a great thing to do in the long run. But I got side effects of such a radical change quickly. The worst was dehydration, my urine changed to the colour of rusty water. I got rid of this very quickly by drinking loads of water, but it meant for a few weeks I had to be near a loo most of the time! Also, make sure you get enough salt. By cutting out the high carb processed foods, your salt intake is going to plummet. We all know about the advice regarding too much salt, but too little can be a problem too. You will know your salt intake is too low if you get a lot of cramps, particularly in your legs.
