Researchers from the Perelman School of Medicine at the University of Pennsylvania have found that an experimental lipid-lowering medication plays a pivotal role in the control of high blood sugar brought on by glucose production in the liver (hepatic gluconeogenesis-induced hyperglycemia). The study findings, published online this month in Diabetes Care, show that the new generation of antisense-oligonucleotides drugs (ASOs), commercialized as volanesorsen by Akcea Therapeutics, can both lower apoC-III - a key regulator of plasma triglyceride levels – and improve insulin sensitivity and glucose control. Data from preliminary phase 2 clinical trials presented recently at the American Heart Association (AHA), European Society of Cardiology (ESC), National Lipid Association (NLA) and American College of Cardiology (ACC) – has sparked substantial interest within the medical community. In patients with high to severely high hypertriglyceridemia (fasting triglycerides higher than 5.7 mmol/L), the apoC-III inhibitor reduced low-density lipoprotein-cholesterol particles (VLDL-C) and increased the protective high-density lipoprotein particles (HDL) while also keeping other risks factors for cardiovascular disease at bay. In this study, research scientists have now discovered that ASOs can also improve insulin sensitivity and glucose control by significantly decreasing patients' overall hemoglobin A1c (HbA1c), as well as improving peripheral insulin action at a tissue-specific level. Previous mice studies have demonstrated that ASOs can disrupt the expression of an important transcription factor in glucose metabolism called Foxo1 in the liver. Foxo1 regulates the gene expression of two key enzymes. The first one, PEPCK, is an enzyme used in the metabolic pathway of gluconeogenesis and controlling hepatic glucose output. The other is glucose-6-phosphatase (G6Pase), an enzyme found mainly in the liver and the kidneys that hydolises and liberates glucose into the bloodstream. In insulin resistant animals, Foxo1-targeted ASO therapy lowers plasma glucose concentration by decreasing expression of G6Pase and the rate of glucose production by decreasing expression of PEPCK. In other words, the decrease in Foxo1 triggered by ASOs decreases expression levels of PEPCK and G6Pase, subsequently decreasing gluconeogenesis and reducing fasting blood glucose. In order to quantify the effects on blood glucose control of the drug in humans, researchers tested the medication on 15 adult patients with type 2 diabetes and hypertriglyceridemia assigned to two groups. One received volanesorsen and the other a placebo. After 12 weeks, the research team found that patients on ASOs experienced both a 69 percent reduction in triglycerides and a 57 percent improvement in whole-body insulin sensitivity, evidenced through hyperinulinemic-euglycemic clamp testing. Their hemoglobin A1c levels also significantly improved. This suggests that ASOs, which may bypass defects in insulin signaling, could represent a possible therapeutic target in insulin-resistant type 2 diabetes patients with high triglycerides.

I've found the LCHF diet is excellent at lowering triglycerides - I haven't had a reading over 1.6 since starting the diet 4 years ago - mostly I'm around 1.0-1.1. It's also raised my HDL substantially.

Lipid Biomarkers (cholesterol, lipoproteins) and Cancer Risk

Lipid biomarkers are frequently used to assess the risk of heart disease. A simple blood test provides information about LDL-cholesterol (LDL-C) (1), commonly nicknamed the bad cholesterol and HDL-cholesterol (HDL-C) (2), often called the good cholesterol. Triglycerides are usually measured as well (3).

For many different reasons, lowering LDL-C has become a primary target for the prevention of heart disease. Evidence suggests a relationship between LDL-C and the risk of coronary heart disease. Lifestyle measures that lower LDL-C are usually recommended, and statins (cholesterol-lowering drugs) are used by millions of people worldwide to lower LDL-C.

Blood levels of triglycerides are also associated with the risk of heart disease (4). Furthermore, there is an inverse correlation between HDL-C and cardiovascular risk. Hence, high HDL-C levels are associated with less risk of heart disease, and low levels are related to increased risk.

But, what about the relationship between lipid biomarkers and cancer? Do we find the same pattern there?

Cancer is the second leading cause of death in most western countries. Diet, smoking, and obesity are known risk factors for cancer. These risk factors often correlate with alterations in blood lipid markers.

Lipid Biomarkers
Because fats are insoluble in water, cholesterol and triglycerides cannot be transported in blood on its own. Instead, cholesterol is attached to hydrophilic proteins that function as transport vehicles carrying different types of fats such as cholesterol and triglycerides. These combinations of fats and protein are termed lipoproteins. The lipoprotein particles vary in the primary lipoprotein present and the relative contents of the different lipid components.

The lipoproteins are classified according to their chemical properties. There are five major types; chylomicrons, very low-density lipoprotein (VLDL), intermediate-density lipoprotein (IDL), low-density lipoprotein (LDL) and high-density lipoprotein (HDL).

Measuring the amount of cholesterol carried by the different types of lipoproteins has become standard practice. Thus, LDL-C reflects the amount of cholesterol carried by LDL and HDL-C reflects the amount off cholesterol carried by HDL.

However, measuring the concentration of the lipoproteins themselves may be more informative than measuring the amount of cholesterol within these particles. There is substantial evidence that some lipoproteins play a fundamental role in heart disease and their interaction with the arterial wall appears to initiate the cascade of events that leads to atherosclerosis. Lipoproteins that promote atherosclerosis and cardiovascular disease are termed atherogenic. Apolipoprotein B (apo B) is a major component of atherogenic lipoproteins (5).

Apo B occurs in 2 main forms, apo B-48, and apo B-100. Apo B-48 is mainly synthesized by the small intestine and is primarily found in chylomicrons. Apo B-100 is the protein found in lipoproteins synthesized by the liver. From the viewpoint of cardiovascular risk, apo B-100 is the important one.

Apo B-100 is the primary lipoprotein in LDL (6) and other lipoproteins that promote atherosclerosis, such as VLDL and lipoprotein(a) (7).

Apolipoprotein A-I (Apo A-I) is the main protein component of HDL. As with HDL-C, low levels of Apo A-I are associated with increased risk of heart disease and high levels seem to be protective.

Apo B, apo A-I and the apo B/apo A-I ratio have been reported as better predictors of cardiovascular events than LDL-C (8).

Lipid Biomarkers and Cancer Risk
The role of lipid biomarkers in assessing the risk of cancer has not been studied in detail. However, lipid metabolism has now been accepted as a major metabolic pathway involved in many aspects of cancer cell biology (9). In this context, the contribution of dietary factors, such as different types of fatty acids, carbohydrates, and added sugars may be of importance. Hence, it is likely that future therapeutic strategies for cancer will include dietary regimes. Blood lipids and lipoproteins may influence the risk of cancer through insulin resistance, inflammation, and oxidative stress (10).

Some studies have found an inverse association between total cholesterol and the risk of cancer, suggesting that low blood cholesterol may increase cancer risk (11,12, 13). This has led some experts to conclude that too much lowering of cholesterol may be harmful.

However, other studies have found a positive correlation between total cholesterol and the risk of cancer (14).

Very few studies have addressed the association between novel biomarkers such as Apo B-100 and apo A-I and cancer risk.

So it appears that the relationship between lipid biomarkers and cancer risk has not been clarified yet. However, a recently published paper addressing lipid biomarkers and long-term risk of cancer in women may have cast some light on the issue.

Lipids Biomarkers and Cancer Risk in the Women's Health Study
Chandler and coworkers evaluated the association between plasma lipids and risk of cancer, in a prospective analysis in a large cohort of women aged ≥45, who were free of cancer and heart disease at baseline. The results were recently published on-line in the American Journal of Clinical Nutrition (15).

Breast cancer, colorectal cancer, and lung cancer are the most frequently diagnosed cancers in women. The association between lipid biomarkers and the risk of these cancers was addressed in the study.

Although apo A-I is correlated with HDL-C, and apo B-100 is correlated with total cholesterol and LDL-C, the authors hypothesized that apo A-I and apo B-100 might provide a superior risk prediction for certain cancers than would standard lipid markers.

A total of 15.602 females were followed for a median of 19 years. There were 2.163 incident cancer cases (864 breast cancers, 198 colorectal cancers, and 190 lung cancers).

After multivariable adjustment, women in increasing quartiles of Apo A-I and HDL-C had significantly decreased risk of incident total cancer. The association was slightly attenuated after adjustment for body mass index. Total cholesterol, LDL -C, triglycerides and Apo B-100 were not significantly associated with total cancer incidence.

Women in increasing quartiles of apo B-100 and triglycerides had increased risk of colorectal cancer. HDL-C showed an inverse correlation with the risk of colorectal cancer.

No lipid biomarkers were significantly associated with increased risk of breast cancer.

Women in increasing quartiles of HDL-C and Apo-A1 had significantly decreased risk of lung cancer. After adjustment for BMI, the correlation was only significant for HDL-C.

LDL-C was not significantly associated with risk of total cancer or any site-specific cancers.

The authors of the paper concluded that lifestyle interventions that reduce apo-B 100 or raise HDL-C may be associated with ireduced cancer risk.

The Bottom-Line
Recent evidence suggests that when it comes to cancer, a high-risk lipid profile is somewhat different from that used to predict the risk of heart disease.

Today, LDL-C is a primary target for cardiovascular prevention. However, this biomarker appears pretty useless when it comes to predicting the risk of cancer.

Low HDL-C, on the other hand, seems to be a strong predictor of cancer risk and so are low levels of Apo A-I, the main protein in HDL.

The findings of the recent study by Chandler and coworkers support a possible role of lipid metabolism in the development of cancers. The authors of the paper conclude that the attenuation of the associations between lipids and cancer risk, when adjusted for body mass index, suggests that blood lipids may be involved in the development of cancer through pathophysiologic processes related to obesity.

Low HDL-C and Apo A-I levels are few of the key features of the metabolic syndrome which is also characterized by elevated triglycerides, central obesity, hypertension, and glucose intolerance.

A recent study in the prospective Metabolic Syndrome and Cancer Project suggested a potential role of triglycerides in the development of cancer (16).

Several observational studies suggest that central obesity (17), insulin resistance (18), hyperinsulinemia and low adiponectin (19) levels are associated with increased risk of cancer. Although the mechanisms of this link remain obscure, it is possible that hyperinsulinemia in itself is important in linking insulin resistance to cancer (20). Insulin is a known growth factor and has been put forth as a causative factor in cardiovascular disease. Also, hyperinsulinemia promotes inflammation, which is an established risk factor for the development of cancer (21).

The association between lipid biomarkers and cancer may imply that dietary measures aimed at raising HDL-C and Apo-I, and lowering triglycerides and Apo B-100 may reduce the risk of cancer. However, due to the observational nature of the study, this can only be regarded as a hypothesis.

Although low-fat diets may help to reduce LDL-C, low-carbohydrate diets are more effective in raising HDL-C and reducing inflammation (22, 23). Other lifestyle interventions for cancer prevention such as regular exercise and smoking cessation are also useful in raising HDL-C.

fer sure but there are a few exceptions so ratios are important, sizes of particles 9fractionated lipoproteins test) are important and whether or not you have what kind of heart problem may factor

If you stick to the classic figures, the ratio's will always be good, low trigs and low LDL will give 'good' LDL, so I keep to the standard advice, and keep to the targets recommended.
But it is interesting to see what else low cholesterol may improve.
I've never subscribed to the 'higher is better' cholesterol theories, and this seems to reinforce my viewpoint.

yes it does substantiate your viewpoint. i am curious what either the intermittant fasting and/or the HFLC diet would do to my nuhmbahz as designed and / or whether either diet has to eliminate fatty meats to optimize lowering triglycerides or whether this matters. There are a lot of people on this forum channeling bacon in their lives and dreams and no one really talking thrombosis, stroke, high blood pressure, age appropriateness, ratio of veggies to meat, any of that. I would like to think that a diet that works for weight like LCHF could be truly tweeked for vegans, or modified for people like a friend of mine with Barrett's esophagitis (or anyone with esophagitis that hasn't reached that stage) who have to follow low fat diets the rest of their lives).

For myself and other people of my religion who for over half the days of the year may not eat meat, fish or dairy and for most of that have to eschew all but seed oils (Orthodox Christian diet), having some kind of vegan version of the LCHF tested and true would be a great blessing . Monks of our religion also do a kind of intermittent fasting, similar to Islamic peoples during Ramadan, where eating at all doesn't happen until evening during fasting periods BUT anyone considered ill, inforim or an active student need not keep fasting rules and diabetics come under this in the Canon Law. So Basically, diabetics get a free pass on the fasting periods which are like the Islamic fasting diet except there is a vegan component. Islamic people get to basically feast after sundown. We are coming from the same traditions since Mohammaed himself used to be Orthodox growing up and both Eastern Christians and Muslims were influenced by Jewish dietary laws and the periodicity of the fasts.

For LCHF to be a successful alternative, it has to be able to be adapted to various religious traditions. For religions like Hinduism, it is especially important

Someone shouted me down on the Low Carb Forum for even mentioning some of this life threatening stuff stating that the first video mentions that you should bring up LCHF with your doc but the forum features very slow to access videos that do not provide all the features of the diet to bring up with said doc until the program is over. To actually produce something to discuss with an endocrinologist, there should be some handy Pdf or other format downloadable black and white doc with all the essentials of the diet if indeed the masses of people coming on this forum are going to discuss and perhaps tweek Mr. LCHF diet with their docs in advance of trying it.

It does appeal, and is a good way of eating.
But as you say, there are a vocal few who seem unable to accept any aspect of it may not fit into your lifestyle, beliefs, or even health.l
I too have seen that.
There are many more things that high fat may or may not be good for, and but simply accepting saturated fat doesn't work for me, as my cholesterol goes through the roof has met some resistance at times.
The fasting during daylight hours, and feasting at night, is something I would consider with care.
I believe it was this way of eating, (due to work pressures, rather than religion) which overloaded my system, and led to weight increase and diabetes.
As you say, the ill and infirm are allowed to eat during the day, but it is considered more respectful to eat plain food, and drink water.
Possibly the way to go would be a basic vegan version that would certainly be more acceptable as the base line for all, then the various add ons could be introduced for those that need to tailor it for themselves, as you say.

But I am wondering whether the ill and infirm should instead fast during the day. As you say, often for religious reasons eating a little in the day is translated into a bit of bread and water or tea which for diabetes can make you feel worse, so sometimes respect has to take a backseat.

Meanwhile, a synthetic Omega 3 stnadardized, Lovaza, has been what I have relied on for several years to lower cholesterol and manage triglycerides but the insurance industry in the US doesn't cover lovaza, now an expensive over the counter unless your triglycerides are astronomical. A pharmacist suggested I find a high potency Omega 3 from wild fish oils to take its place at about a quarter the cost of the voer the counter generic of Lovaza once my last three month stash runs out. I can attest, hthrough tests, that the Lovaza has worked for me in the Cholesterol situation and for Triglycerides and have even experimented on myself with the number of pills a day and what effect that is on the tests. I was taking 4 a day optimally for good numbers. Lowering to two jacked my numbers way over a 100 upward in the Triglycerides, 50 in the cholesterol

Education, and the internet are now working wonders.
Bread and water can be replaced by better foods, low in carbs, but not high in luxury.
Many vegetables can now fill the role.

And as you mentioned the Jewish dietary role, it has always been a rule that chicken soup will cure any illness. :)
Fish is a staple for me, but there is a danger from farmed fish now, and the recycled contaminants that are in the feed.
I think it is interesting, how much is being learnt about nutrition, and how it is affecting every aspect of our lives.
The old rigid dogmas are being replaced, and the new rigid dogmas are being questioned exponentially now, so we are moving on, and progress is always to be welcomed.
