Water fasting and medication e.g. Metformin

Having to forswear foods I have loved all my life.
Trying to find low carb meals when eating out.
Just a quick question.

If you are on medication such as Metformin (which should normally be taken with meals) what do you do if you are on (for example) a 24 hour water fast?

Skip the medication, or risk it on an empty stomach?

I assume for the more active medications which carry the risk of a hypo you would always skip the medication.

Read the Patient Information Leaflet (PIL) My variety of metformin advises NOT to take them if following a VLC diet of <1000 cal

I'm still just over the 1,000 kcals at the moment (without water fasting)

I just take my metformin as usual. My food intake goes up and down from almost nothing to 2000 calories or so a day. All very low carb.
