So hi again!!! I ve been on basal now for over 3 weeks and my sugars have stabilized to an extent!!waking up between 5-7 every morning but my problems are meal times,i m less than 100 carbs a day and my sugars are ridiculously spiking to high,I rang diabetes nurses today to explain my situation,AGAIN!!! and told her that I feel as though should be bolusing for meals due to my sugars been to high and wanting the best control,i told her they were 15s 3hours after meals 10 before next meal she was talking about putting my basal up! I think i should be bolusing for meals,i told her.

Heya - agree you should be on quick acting insulin for meals, you need to call them back, tell them you need to be bolusing for meals, getting those levels back down into range should be a priority. If they still disagree tell them you will be making a complaint regarding this advice, sounds like it's time to get tough :)

I'd ask them why they are not following NICE guidelines and not giving you a bolus insulin as it is obvious you need it. Unless they are of the thought you are in fact Type 2?

I was diagnosed with type 1 2 years ago after been omitted to hospital with dka, after that I went through a honeymoon period which I m no longer on.I had no antibodies on diagnosis which confused the lot of them, just had another antibodies test which came up abnormal so theres the proof I think,I ve an appointment with the consultant next month who will be getting a tongue lashing for sure,i m not an experiment!!!

The diabetes nurse you spoke to is that at your local GP surgery or at your diabetes hospital clinic? Usually a GPs DSN are specialist in dealing with T2s, metformin and pushing the eatwell plate, if it was a hospital clinic DSN I'd call them back and demand an appointment with a consultant this week or at least a phone call from one.

I agree with Slip - my immediate thought process was the DSN thought you were type 2 hence the basal advice, in reference to NICE this summarises perfectly why you need quick acting insulin:

Blood glucose targets
1.6.13 Advise adults with type 1 diabetes to aim for:
  • a fasting plasma glucose level of 5–7 mmol/litre on waking and

  • a plasma glucose level of 4–7 mmol/litre before meals at other times of the day. [new 2015]

I can't believe the NHS Shuffle you're getting. No bolus only basal, ***? I mean that could turn out bad at times. I'd march over to wherever and tell 'em you're not leaving without a box of Novolog pens. And BTW your photo looks like a young Kiefer Sutherland. Right?

You could try not eating food that raises your blood sugar. Just a thought At least until they give you bolus insulin. Eat high protein meals with meat cheese and low carb vegetables.

Oh my goodness, your situation seems crazy! I'd phone the consultants secretary and insist on an appointment immediately to get it sorted. Failing that, eat ALL the carbs and turn up to A&E with HI sugars. Good luck.

If I was you I'd harass them until this was resolved. Once they realise you're going to be a pain in their butts until they take you seriously they should hopefully sort out the bolus insulin you clearly need.

I'm sorry that you're just being fobbed off right now, I know how it feels and it's incredibly frustrating and annoying.

I'd also be tempted to try and get referred to another Consultant because the one you have sounds like a right plonker! I'd also seriously consider just turning up at the diabetes centre in the hospital and trying to get someone to discuss things with you. It's a lot harder to look someone in the eye and fob them off than it is to do it over the phone.
