Tresiba not reducing fasting bloods after one week

Hi folks

First post. I have been insulin dependent Type 1 since 1992. Only been looking after it in the last five years as never accepted it. I have now having lost the sight in my right eye. I'm very positive about everything.

I attended a carb counting course with my specialist team in June and it did wonders for my levels. Everything went into single figures for the first time ever. Felt so much better.

I was taking 60 units Lantus at bedtime and one thing they wanted to try was to move me over to Tresiba 100. It was hoped that the fact that it lasts for approx 36 rather than the listed 24 of Lantus (some reports say shorter) would allow better control and over time reduce the number of units.

A week in - so well past the 'steady state' which I have been told can take 3 days and every single fasting blood has been in double figures - most of time close to 20!!!

Anyone out there have had a similar experience? I'm tempted to go back to Lantus.

Before moving to Tresiba I was hopeful that my HBA1C would have reduced below last Julys 73 (I know I know - way too high but working on lower this year) .

Any guidance/help would be great

We have some Type 1s here on Tresiba so hopefully they'll see this and reply :)

How many units of Tresiba are you taking?

What are your levels when you're going to bed if you're waking at 20? No basal, Tresiba included, is going to lower fasting readings if you are starting off high. The job of the basal is to keep you flat. So if you're going to bed at 18 and waking up at 20, the basal is doing its job and keeping you flat, just flat at a higher level than you would like and that higher level should be dealt with with a correction bolus. If you're going to bed at 7 and waking up over 20 then that suggests the basal dosage needs adjusting & it would be worth discussing that with your DSN.

It took me about a month to get my Tresiba dose correct and my body used to it.

I took a little less than a week to taper off Levemir and on Tresiba. About two weeks in, I actually almost gave up on it and wanted to switch back to Levemir.

However, it's a different insulin and behaves differently. I gave it a chance, nailed down my basal rates and haven't looked back.

I'd say more than half the people I've read about are like you and me where they had minor frustrations in the first month. The vast majority eventually got their doses right and are very happy with the switch.

Thanks for the replies. Using the forum via the iPhone app and haven't got the notifications working.

I was on 60 Lantus at bedtime so started taking 60 Tresiba.

Started a new batch on Saturday night just in case.

Last I scanned my Freestyle Libre at bedtime (2314) and it said 3.0 ️ (showing stable) but I felt fine so I did a blood strip in the same meter. It came back as 4.6. No carbs - no snack. Just my 60 units of Tresiba.

This morning - 19.4. This was using blood strip as I can only use the carb counting/correction element of the Freestyle Libre (setup by my DSN) that way. Correction does of 13u NovoRapid. My carb ratio is 1:6 rounded to 2:10 and my correction dose is 1:1. Bit h calculated with DSN after weeks of constant testing and careful carb counting. Those ratio worked pe deftly with Lantus.

I will keep with it for a month and see how it goes. I was so close to being in range almost 95% of the time but seeing these high tastings BGs was a little frustrating.

PS I have emailed my DSN this too and waiting to see her thoughts. I will m toon the suggestions of 3-4 weeks.

If you are on Libre then are there any clues in overnight profile?

Hi

Only put the sensor on yesterday and didn't scan overnight. If I check the Daily Patterns graph it just shows a slow climb to the peak to when I took my BG this morning.

I'm almost pretty sure it's not low GI carbs as the last thing I ate was at 2100 and it was white bread, toasted - so high GI. Took enough NR to cope and levels came down accordingly.

Right probably best to let Libre run a few days and see how it looks over the day before deciding strategy to tackle the overnight rise then. My experience is the other way with BGs dropping between 8-12 hrs. Whether that is Tresiba or just me, who knows!

Morning folks

Just a quick update. It's all working really well now. Fasting BG are in range, for the most part and where they are not I know why.

Also, having completed the local SHINE course (just like DAFNE) in June, yearly HbA1C was down 10%!!! Delighted. Another 20%- 30% to go but I am happy with a 10% yearly reduction as I do not want to cause any long term affects of more of a reduction.

When I first moved to Tresiba back in December last year, I was hoping for a miracle after the first week. Sadly, it never happened. It took 3-4 weeks for my BG to fall within range but since then my levels have been amazingly stable. Give it a couple more weeks. I know it's frustrating but it's worth the wait.
Is it good, the hospital I go to might put me on that but I do not know. I probably won't have a say on what I'll take for a new long acting insulin.
