High blood sugars and lactate

I've been suffering with high blood sugars and cannot get them down can go from 12 to 26 more so in twenties.there isn't any pattern to it either.j was on metformin but after being sent to a & e by gp with added 4+ ketones with suspected DKA they took me off it and replaced with gliclazide.
Fast fwd a few weeks after another 2 overnight stays in hospital due to high blood sugars and lactate I am now finally in touch with community diabetes team,who are trying so hard to help me,but bloods sugars still unstable.? I have been started on insulin and having to up it every 4 days as advised.But just wondered if anything else I can do diet wise etc.. I am counting calories but so confused with carbs and ketones its stressing me out.

Hello and welcome,

As someone being type 2 and only on metformin my advice is not as helpful as some others- however,

What advice have you been given about diet? Most type 2s find that by reducing (dramatically ) their carb intake they can reduce or eliminate blood sugar lowering medication. If you are taking any blood sugar reducing medication eg insulin (and glicazide?) you will need to be a bit more careful.

maybe give us some information about what you would typically eat in a day- we can then give you some suggestions.

I am sorry you have not been given sufficient advice by your health care professionals -Un fortunately it happens more than you would think. The good news is that this site is made up of many diabetics who are awesome and ready to provide support and guidance- and- we have lived it.

So welcome and I'm sure someone more knowledgeable will be here soon.

Hi thanks for your reply.
I haven't really been given any advice re diet, just get told I'm overweight which I'm not disputing .I have lost 9lb since march but think maybe the ketones have played a part in that.But gp was questioning if I was eating enough carbs??
This is where I'm so confused with it all.ive download my plate and track my protein fat and carbs daily.

That’s way too carby for me. I’ve been under 20 for the last few months, which has helped lower my blood glucose levels.

Edit, I just noticed your calories, it may be that your metabolism is low. I’m targeting 10% under, adding in exercise, trying to stay active

I'm another who has way less carbs than that. I am usually under 10 grams a day but definitely under 20 grams. I've lost weight and blood sugars under control.

as you are on blood sugar reducing medication you need to be careful and test often. Try reducing gradually your carbs and see what effect that has on your levels. But test regularly- it is important.

Hi there, all of the above comments are well meaning but I would say the first thing you need to do is get a diagnosis of what type you are. The suspected DKA episode & hospital admissions may be indicating type 1 and either way they have decided to put you on insulin. All this talk about 'too carby' at this stage is very misleading, you are still under observation and any drastic changes in your diet could well lead to further misdiagnosis. DKA as you are aware can be extremely dangerous and THAT is what you should be focusing on not whether you should have weetabix or whatever for breakfast. Don't get me wrong, diet is very important too no matter what type you are but at this stage I would be asking for further tests (if you haven't already had them) and further information with regard to why you ended up in hospital, etc, etc. Once you are in a position where you know whether it is type 1 or type 2 you can then adjust your diet accordingly to your wishes. Have you been started on a fixed amount of insulin?, that may be why they are saying you need to eat more carbs to match it, if you remain on insulin eventually they may change you to flexible doses but to reduce your carbs as suggested above whilst you have no idea of what is going on is bad advice. Please speak (every day if needed) with your team. x

But you are type 2 and the Poster's situation is unclear but they are on insulin so any carbs/exercise and so on is only part of the solution and at this stage a small part at that.

It's only this last few month I have started having problems with this.Everytime I go into hospital in a & e they say something not right here,to keep causing lactate levels to raise,then ward doctors say uncontrolled diabetes off u pop home.
The community diabetes team have been gr8 only been involved this last 3weeks how have now referred to a specialist for testing a rare form of diabetes. (Genetics)

Hi RacRob, I am sure they are right but just keep an eye on it all as they do make mistakes. I was said to be 'pre diabetic' in 2017 and 3 years later...oh wait...it was type 1. After a chaotic introduction (DKA & hospital!!), I was placed on insulin. x.

Hi thanks for the reply!
My optician flagged up in November re early eye retinopathy, told to ask gp to recheck hba1c levels as her view of my eyes was not someone with diabetes for 2 years but more like 10!!
Which I did it's been checked 3 times since and has been creeping up but reported as abnormal to be expected. Then on highest dose of metformin 2 x daily.
Hospital since taken metformin out of equation re raised lactate now on max gliclazide 2 x daily and insulin pen.still so far not touching blood sugars.

Hi is they any one on a Libre freestyle when I Scan my libre it’s telling me in hypo and when I am still pick my finger with my finger pricker it’s telling me I’m 6.6 dose any one have the same thing as me
