Tresiba basal insulin

Hi everyone
I'm type 1 and was diagnosed 7 years ago. I was on Levemir but my blood sugar were always high. I'm only the 5th person at my hospital to be put on Tresiba. It lasts for up to 3 days in the system so if I forget to take it one night there are no bad effects.

My problem now is that I have a hypo every morning and have done for the last 7 days. A good thing is that I hardly have to take any novorapid. In one week I have only had to have 5 units. Its unbelievable and my diabetes nurse is amazed at how good its been for me. Although I'm having to tweak it to find the correct dose for me. This morning I woke at 4am with sugars of 2.1 Its taken a lot out of me this week.

Does anyone else take Tresiba?

Yes there is another type 1 who takes Tresiba insulin, I'll 'bump' your post up in the hope that you get a reply.

I hope that some good vibes come from using tresiba. Not good that you are hypo though at 2am. Have you tried eating some carb without the bolus before going to bed to make yr bg levels go a bit higher as a way to off set the drop?

I've ranged from 8 up to 12 before bed and have had a couple of biscuits and milky drink I still wake in the night or early hours with a hypo. I'm on day 16 of hypo on the trot. I'm exhausted Diabetes nurse says to keep reducing tresiba. It's in units of 2, I've gone from 16 down to 10 gradually and without any novorapid for last few days. It's all very strange.

my DSN today agree that i could benefit from tresiba, but needs to get approval from my consultant... I'm currently on Humulin I, as i had issues with levemir and lantus

be interesting to see if i get it

I have just been prescribed this newfangled basal insulin. :cool:

Does anyone have any insights to share about switching over (I have been on Lantus) and/or using this?

Start with a smaller dose of maybe 2/3 of your other insulin, don't forget it's longer action of about 3 days means it build up in your system so wait about 3 days before adjusting your dose

You will have less or no post insulin lows as it has a very smooth profile

Take every night before bed and it's not too time sensitive on when you take it

Thanks searley, that's really useful to know. I'm off to Spain in a few days, so will wait until I get back before switching over.

I started using it 3 weeks ago, so not much useful info to share, yet. But first weeks have been pretty good. Have been measuring the bloodsugar every 2 hours during the day and 95% of results have been between 5 - 8. I take it in the morning, easier for me to remember to take it.

Thanks marilee - good to hear that your first weeks and levels have been good. I know I said I would wait, but was too impatient and started yesterday. No probs so far :)

I was one of the first diabetics to take tressiba over a year ago. I still take tressiba if i have a break from the pump. Sometimes I take a weekend off my pump. I would take 23 units at the same time every day. i found that it was important to take it at exactly the same time each day or id get hypos. For me, I took mine at 1pm every day. That way I woke up with fasting levels of 6-8. My diabetes specialist said it was important to take it at the same time each day because its peakless.
If you keep having hypos, Id recomend to reduce the dose by 2 units each day untill the hypos ease off.
I also found that a hypo on tressiba was not as intense as a hypo when I was on Lantus. Do you find that? I also found that I recovered faster.

When I first started on tressiba, Id take it in the morning upon waking. My fasting bloods were about 9-10. I then forgot to take it one day and realised that I had forgot at about 1pm. I then took my Tressiba. The next morning, I woke up and was 6. I never wake up with a reading less than 9. I decided to take my tressiba again at 1pm. the next day I woke up with a 5. The pattern continued so I learned from my own experience that if a person has dawn phenomenon like me.....then change the time you take your insulin.

Thanks Omnipod, useful info and I will bear that in mind. I'm currently taking it at 6pm as that's when I took my Lantus.

It's only my second day, so it's still a bit of an adventure :playful:

@ Robert... HBA1C 5.9%. Wow. Im impressed. Awesome and well done. I have never got below 7% in 15 yrs now. Im trying.

I've changed the dose in 1 unit steps from 10 to 14 and back down to 12 and switched from taking it at 11pm to 8am but to no avail. I wear a Freestyle LIbre so I can see exactly what's going on 24/7 and some nights I go high then come back down in the morning, others I go high and stay high all day no matter how much extra Novorapid I take. I'm really fed up and feeling increasingly unwell. My last HBA1c was 9.1 but with all this mucking about I'm expecting it to be over 10 next time round. BTW I'm on 500mg Metformin as well.

I'm going to ask my consultant (if I ever get to see him) whether I should go back on Levemir. Has anyone else had problems with this new insulin/advice to share?

I'm finding it extremely flat but you must get the dose right. Try doing some basal testing and then start re-working on you bolus ratios.
