Hey guys,

Can I ask for some advice please?. Since Thursday night (11pm) I've been running between 12 & 18mmol/l and today I'm feeling sick and I have a sore head. Ketones are 0.1, so they're okay but I'm not sure what to do. As I'm obviously still taking my insulin, does that mean I won't get any ketones?.

I don't know how to basal or bolus, so in terms of giving a correction dose, I'm completely lost :(.

I'm on 2 units of levemir in the morning with 9 units of humalog. 4 units of humalog with lunch and 10 units with dinner. Diagnosed for 6
months.

If you can get hold of your DSN (diabetic specialist nurse) she/he should be able to help you better since I would hope they could at least pull your notes.

Do you adjust your Humalog dose according to what you eat or are you on a flat dosage and then fiddling your diet to fit?

The first thing you need to do is to try and ensure you keep your fluid intake up, with high sugars like that you will be dehydrating a bit.
You could try to add a unit or two onto your mealtime doses and then just monitor them to see what effect they have. That will help you to get an idea of what the extra units will do to you.

To help get you started on working this out perhaps you should have a quick look at the BDEC online course: http://bdec-e-learning.com/

If you could also give us an idea of what food you are eating to go with each of those doses and what your BG is just before your next meal that could help paint a clearer picture.

Seriously dude why didn't you go to see or call your Doctor or Diabetic nurse,

I don't know how things work in the UK but 6 months diabetic and you still do not know how to control your Blood Sugar levels

I hate frekin Diabetic Doctors they are ignorant and irresponsible

P.S. Do not let the idea of Ketones freak you out

First fix your medication dosages and control your Diabetes

Thank you Andy and Jack, I can't get in contact with my dsn because it's the weekend. I've been drinking lots of water today though. I'll keep an eye on it and add an extra unit onto my dinner dose :).

And Josef, I do know how to control my diabetes, which is why I'm asking for advice now, because I'm always between 5 & 7. However, I'm on a set dose and I haven't done DAFNE yet, so I don't know how to carb count properly or do correction doses.

then I would get to the hospital and let them manage it for you, as you are really running blind and it shouldn't be guessed at.
failing that read the sick day topic I linked, although you don't have ketones the high BG is more than enough to run the rules.

I think she did a predictive typo from the phone app she is using...it should have been 'don't know'

Hi Samantha

Based on the 100 rule, and looking at your total daily insulin (25u) your correction dose should be that 1u of Humalog will bring you down by 4 mmol/l. You might need a bit more when you're up in the teens, but it's a good starting point. Be careful not to take another correction dose within 4 hours or you risk stacking insulin and could go hypo.

Six months in, it could be that your honeymoon period is coming to an end. You're obviously on top of your testing so keep it up and call your DSN on Monday. Hope you get sorted soon.

I would also get new vials out of the fridge, just incase one of them is off

Hi Samantha,

Until you get a DAFNE course this online BDEC course (http://bdec-e-learning.com/) is fairly similar and teaches you the same basics in terms of carb counting and corrections etc.
It doesn't cover doing basal corrections which can be a little more fiddly but it will give you enough to get you on the right path and give you a bit of a head start on some of the principals they teach in DAFNE.

Robert seems to have hit on the money, one thing I have found is that once my numbers hit around 12 / 13 I need to tag on an extra half to 1 Unit to my corrections to get them to work.

further to what Robert said when he worked out your correction dose, on page 8 of the second link I put up to the workbook, it has a handy 'Correction Bolus Calculator' chart

Robert saved me lol, I took two units and it came down into the 8's but now before bed I'm 15.2! But I developed a temperature late on tonight, so I'm probably coming down with something. No ketones though! So all is good.
