Hi, sorry to bother you but I am really stressed out as not sure how to complete DLA forms for my 9 daughter who has type 1 since age of 2 but never claimed. However now I need help and scared of competing forms myself with the fear of refusal. Our nurse said she cannot fill the forms for us. I came accross your post and would really appreciate it if you could send me a copy of forms to use as a guide if possible?
