Different pens allow different maximum units, the reusable Novopen Echo for instance does increments of 0.5u and it's max dose is 30u, we all have different insulin sensitivity and those with insulin resistance do tend to require larger doses

The DSN isn't to know the amount of carbs you can tolerate as the individual you are so they can hardly tell you how many carbs to eat, only you can find how many you can tolerate through testing

Which Type are you? It doesn't state under your avatar
If you are Type 1 then yes they should but if your Type 2 then they don't have to as Type 2's are less likely to ever develop ketones as they still produce insulin
