If I eat more carbs than I should, I typically get high readings for around 3 hours, then suddenly back to normal by 4 hours. I don't take metformin, or any other medication, so I assume this is the 'second phase' insulin response kicking in.
I think you're going to extremes - that may work for some, but many will fail with that type of approach. I still have milk in my tea, I just use less than before (less than half as much - the tea tastes no different). And a small amount of pineapple now and again isn't going to cause you any...
You need to check the label for the carb content, as they vary quite a bit. I use Tesco finest coleslaw because it's lower carb than most (2.3g per 100g), and it tastes good. It's relatively expensive for coleslaw, but still relatively inexpensive compared to a lot of other food.
Sounds like you are fed up with grease rather than fat, which is understandable. But there are other types of fat - nuts and cheese for example. Also, you don't need to have loads of fat, just enough to stop you getting hungry.

Watch the berries, nuts and yoghurt. They're lowish carb, but only in small portions. Perhaps try cutting back on those and see if your BG comes down some more.
Edit: Also noticed you mention tomatoes a couple of times. Again, watch the portion size and frequency on those.
