Is it possible to have successful 2nd pregnancy with eye issues?

Hi and help!

I have a two year old som and desperately want another however I had an extremely difficult first pregnancy with my eyes and also other bizarre happenings!

The very rare and strange things I went through were adrenal insufficiency (undiagnosed until 4 months post partum) which was very debilirating and then third nerve palsy which occurred at 32 weeks. I've been told these things were so rare it's unlikely to occur again. I also have some degree of autonomic neuropathy apparently. however my eyes are the main issue I am worried about.

Because my sugar levels were too high when I discovered I was pregnant, I brought them down very very rapidly, I think it's quite common or is often a huge reason why background retinopathy progresses so quickly. My eyes went through each stage until I lost most vision, ultimately I had a successful vitrectomy and things did improve around 5 months post preg. The other issue I had / have is the macular edema, this has continued two years on and I'm currently getting injections to reduce it (urgh). I can drive and work so it is more than manageable at present, but I'm trying to find out from others whether it's possible to have a second baby with all that has happened. This time I would be planning for it and have a healthy hba1c so there wouldn't be a sudden drop in levels, but unfortunately I do have damaged eyes so hmmm. I had hoped the macular edema would have been more stable / gone by now as I'd like to be in a more stable place before conceiving.

aNY experience of this and your outcomes / advice you've personally been given would be really interesting to me. My endo is happy, but it's all about the eyes really. My main eye doc said its unlikely to occur, but another has said it will definitely get worse - so conflicting views.

I don't think anyone can say for sure what might happen to your eyes. When I got pregnant I was surprised at how much the eyes could be affected. Mine were fine but the lady checking them said she'd known people suddenly get problems in pregnancy.

It's a hard call for you, especially when you're getting conflicting opinions. I think I'd be asking for the worst possible scenario, the chances of that, the chances of further degeneration, and if there was anything I could do to help reduce the risks.

Thanks azure, they are good questions to ask. I'm trying the noctura mask soon too so hopefully that will help.

I never had any issues but then with my second was seen monthly due to background retinopathy. That's been my biggest thing putting me off having another one as it was scary being told how much pregnancy can affect your eyes! Sorry no advice or experience but I would definitely speak yo both your duabetes team and opthalmology team for advice. Also any improvements your able to do in the meantime to tighten control I'm sure would be brilliant.

Sorry but may I ask. Is it important to have my eye check upon pregnancy? I am 7weeks and didn't have any eye checkup since I got pregnant. I did an eye check up annually which on every January. Should I have my eye check as soon as possible or should I wait for my next appointment on January?

It's recommended that pregnant ladies withndiabetes have extra eye checks in pregnancy as pregnancy can sometimes cause eye problems.

I had two or three checks during my pregnancy. Now I've had my baby, I'm back to my annual checks.

Do speak to your diabetic nurse or your doctor and ask about extra eye checks. If I remember correctly, I think mine were organised after I had my first appointment at the hospital. They notified the eye service and they contacted me.

Hi, yes it's really really important. They should organise for you, but if they don't I would follow it up. I can't remember when I first had mine when I was pregnant- I think I was about 3/4 months and had noticed a difference.

Hi and sorry, I have no useful advice/experiences on this but am similarly interested in hearing what others have to say.
I asked about 2nd pregnancy with existing eye issues a while back but didn't get much - a recommendation for the eye mask tho. Are you about to start using it? Did the docs recommend it for you or how did you end up with it?

My eyes are very well lasered with no more room for further zapping. Some had been done before pregnancy, little bit during (3rd trimester) and a lot afterwards. Now they are finally stable (baby is 14 months). Pretty good levels since 2012 when had a drastic improvement after getting the pump.

The views from the eye consultants have been varied like yours re 2nd pregnancy but main one at my hospital thinks I should be fine. Other one was less positive 'there are always risks'... Guess asking worst case scenarios is a good one. Sigh.

Hi pitsgate, it's so frustrating not getting definitive answers isn't it! Different consultants say different things Don't they?

I'm similar to you with no more room for laser, the retinopathy seems to be ok, but it's just this macular edema which I can't get rid of. Do you have that too?

I found the mask online and on here @himtoo is also using it with good results! I have my appointment tomorrow so will let you know how I get on. X

Hi Emilyprice,

I'm currently 17weeks preg with my 2nd. With my 1st I already had a bout of laser sessions and when I found out I was preg (wasn't planned) I had a load more laser sessions in both eyes. After I had my 1st in 2013, I didn't have many problems with eyes but did have a bleed in my right eye that didn't go away and kept bleeding. This ended up with me having a vitrectomy and cataract operation in the right eye- was told after the op at a 3 month follow-up that there was swelling to the retina and they would keep a watch on it, left eye was ok, this was all last year. So now I'm pregnant with my 2nd (again wasn't planned and had the coil in!!!) had the typical eye screening at 16weeks and due to me being type 1, pregnant, and my history I was referred to the hospitals eye consultant. I had my appointment yesterday and all was ok with my eyes, will be having a follow-up with him just to keep a check on things. So, I've had no probs so far, considering my history and the fact I wasn't even planning another yet. My pre-conception hba1c was 59mmols, and at 15weeks it was 42mmols. I have not been told by anyone that getting pregnant again was a silly idea what with my existing eye problems etc, or that I'm going to have a whole truck load of much, much, worse problems (unless I don't keep my levels in control), it's been the same advice that pregnancy can cause eye problems or speed up existing ones. OP I would imagine that if pregnancy was going to be such a big problem for you/us then the eye specialists/consultants that we've seen or seen along the way, or even the diabetes consultants would have said something after having a child already - that getting pregnant again will cause us, or our eyes real big trouble; unless, as I have already said above that you abuse your levels whilst being pregnant. I'm in similar situation. I'm a diabetic for 26 years now , I had laser on both eyes now I'm getting injections of lucentis (both eyes) it's good , however I can't get pregnant when I'm on it , but I'm desperate to become a mather. That's why I will fellow this post hoping someone could give us hope. I've been offered another injection -illuvian my doctor said that I can take it and be pregnant.but...
I really hope u be fine and don't gave up

I recently had my check-up (about every 3 months) and the very nice doc i saw this time was very positive re my eyes and possible 2nd pregnancy. The way he explained it kinda made sense, tho i'm still not entirely convinced by it all but nevertheless am noting down here what he said (bit theoretic for others but may be of use...?).
Regards the retinopathy - the disease has beginning, middle & end (heh, like all good tales) and mine is now at the end as there is no more space for new blood vessels or lesions or anything, thanks to the very extensive lasering... So 2nd pregnancy would be fine.
Tho he slighly skipped over the retina detachment thing. Or i blinked and missed it.

Macular oedema - or intraretinal cysts - are there but not clinically significant (yet) and are being reviewed...

In any case - he had a good look at it all, and made me feel like it is a risk worth taking, a baby...

How are you getting along with the mask Emily? (I meant to ask the doc about it but seems i have to write down all the questions if i want to remember to ask them!)

Hi all, your posts are so inspirational and I feel so much better that I am not the only person who is suffering with macular edema during pregnancy. I'm currently 23 weeks. And it has been hell. Completely unplanned I was told 8 years ago that I wpuld never be able to conceive. So unplanned and completely shocked. I first got diagnosed with macular edema at 19 weeks it was just in the left eye by 20 weeks I had It in both eyes. My vision is moderate in the right eye but can hardly make things out with the left eye. Iv had one session of laser and it made my left vision worse so I am concerned with having it in my right eye. I feel a bit of a failure atm I'm off sick with work, iv had to drop out of university as I cant see my work or drive to get there. It has come down to the point where I have been considering putting my baby up for adoption when she arrives as I'm convinced my vision will get worse and I want the best for her not being able to see her I won't he much of a mum. My doctor has refered me to a councelor but I really don't see a light at the end of the tunnel atm. I have contacted a clinic that I can but the mask from I am hoping I will have some stabilisation with that. What I was wondering was what are your experiences of macular edema progressing during the pregnancy? Did it spontaneously recede after birth? Did you regain vision? Any information that anyone would like to shair would be really appreciated as my doctors have been vary vague and havnt given me much to be positive about. I'm sure the pregnancy hormones arnt helping with my emotions. But hearing off others that have gone through this would be amazing. Just knowing that some of you are planning on a second child makes me feel positive. Thank you all in advance xx
