Really Need Help & Advice!!!

I was diagnosed as having diabetes September last year as a 41 yr old male. I had lost 7 lbs within 3/4 weeks, from about 12 stone 3 lbs to 11 stone 10lbs. Frequent urination, thirst, etc. Nobody would have described me as obese at the time, height 5ft 10inchs. No hereditary factors. My diet had been generally healthy.
Since diagnosis, prescribed Metformin, 1x500mg per day. I cut down on carbs such as bread/pasta, sugary foods, etc. In late 2008 Consultant said that LADA diagnosis probably most accurate. Since then have continued to lose weight...now weigh 10 stone, 8 lbs, about the same i was when I was around 16 years old.
I took a recent HbA1c test on 17.02.09. with the result at 10.2. For the last 4 weeks I have been taking approx. 2500mg metformin per day, 1000mg at mealtimes (self medicating!!). My diet is very, very low GI and about 1200 calories per day. I eat hardly any carbs at all. I am still losing weight and increasingly tired. Despite this my fasting blood sugar tests are on average about 12 mmol. I can only lower this by running every night after work for 45 minutes. This gets my blood sugar level down to just below 6 mmol, possibly below 5 mmol if I run for 1 & 1/2 hours. Nevertheless, despite this, average blood sugar mmol per day approx. 10-12.

I am due to see diabetic nurse next wednesday. I am used to being in control and able to manage, however I don't know what more I can do. I cannot eat any less, or exercise any more than I do...running every night for 45-60 minutes after a 12 hour day at work is hard on a 40+ yr old, especially on the little i eat.

If honest I am truly scared. I can't afford to lose any more weight, or continue eating as little as I do. Please help
 
Go see your doctor asap and discuss this with him/her.Additional tests can be done to find out the state of your beta cells in your pancreas.
 
Hi Willwawn, and welcome to the forum. What you are describing are the classic systems of LADA, and as your GP as now diagnosed you as such, along with the systems you have described, it will probably be necessary for you to start taking insulin as part of your treatment to control you high blood sugars on a daily basis, as metformin, which only stimulates insulin production will not be working in you case.

Speak to your GP as soon as possible about this, there is no need to have to put up with high blood sugars, plus keeping your bs high, above 10.0 mm/lt will lead to keytone production, cause by your body feeding on it's self, because of lack of insulin production. This is a dangerous situation to be in.

see your gp as soon as, you may need some better medication, but other people on here would know more than i do!
the best advice i can give is the best advice i have been given by the good people here on the website,
try to eat 4 to 5 small meals rather than one large meal, try not to eat a large meal at night! When i first came on here last week i was so scared, i am still waiting on my tests,
since taking everyone's advice i have got my blood sugars during the day below 10, on the odd time 11 plus, but the average between 7 and 9.
i have done this through eating smaller meals rather than one big meal!
there are lots of helpful people here great for advice!
i found it very hard to eat smaller meals through the day! stress will increase your blood sugars!
do you work in a stressful job? if so the stress can cause your blood sugars to rise,
i found a great way to eat a healthy meal, i get a chicken breast and cook it in a teaspoon of olive oil, make it chargrilled then do some veg any sort! then when the chicken breast is cooked pour over a little freshly squeezed lemon, it is nice and easy to cook!
it sounds like you are on the go at your job and alot is expected out of you! do you ever binge biscuits as i have been reading labels, i am trying to keep carbs below 50 or 60 a day!
if you don't eat alot how about a light snack, like some tuna salad, rather than putting mayo with the tuna, to give it a bit of taste add a drop or two of vinegar give it a little tang!
 
hi to you - i really sympathise with you. I have a similar but not quite as drastic situation - it is very worrying. diagnosed last May - already slim, fit and good diet -altho' I had been eating far more sugar than usual before diagnosis - my final fling with the wicked stuff! Couldnt control bs with diet alone. Now on 850mg Metformin twice a day , but weight very low - my arms and legs wasting - down from size 12 to 8 - and only 8stone - used to be 9.5 - crazy! My doc doesnt seem to be worried about this as it's been stable for 1 year now. Last HbA1C was 7.8 but bs often very high for no apparent reason and hours after eating a healthy meal. I have to eat very small meals especially at night as I'm bloated and have all sorts of nasty gastro discomfort. Doc sending me to see gastr-intestinal specialist but I think probs related to diabetes or side effects of meds. I try to exercise a lot but not as much as you. I really wish you good luck and if you get some solutions do let me know!!

With that diet, and amount of medication and still running that Hba1c plus the weight loss - you need to be on insulin pronto

Get to your Gp asap and dont accept anything less than a referral to a hospital for someone who knows what they are doing

Thank you all for your responses, been feeling so alone with this I've found your words (and time in replying!) very supportive.

I'm off to see the Hospital's Diabetic nurse first. She seems more knowledgeable & supportive than GP, who told me on initial, vague diagnosis that I would probably live for a maximum of only 10 more years (I'm 41 yrs old, bmi then approx. 23!) with the strong likelihood of blindness & digit/limb loss! She's commonly known by many as Dr. Death around these parts. I'd rather see anyone but her.

Jeez which medical school did these doctors go to!!I'm sorry that you have been told this,how upsetting for you.
 
Sandy...by the way you say you only posted on the site a week before but you seem extremely perceptive or intuitive. Love my job, though it is stressful... I'm Headteacher of two schools and a children's centre, and incidentally, & probably more stressful, a new, first time father!

Congratulations on becoming a dad!!that makes that doctor's comments even more horrible,imagine saying that!!Makes me very angry.With good control you will be around a lot longer than that.

well done! I usually can tell from the way someone writes as to what is going off! i was training to become a pyshartic nurse!
I always try to offer advice where i think it would be helpfu! you sound very stressed! a headteacher! I must admit that being a headteascher will be very stressful! must be very rewarding when you see the children achiveing things that most children think they never could!
I don't know much about being diabettic, as still waiting on tests!
I hope that everything goes well! congrat's on baby! and congrats to your partner! with everybody's help on this website, and there are lot's of very helpful people who know more than i do.
they will help in any way that they can!
