Wow! That is a much higher dose than what I need to take. I take a single 25,000 Creon capsule with meal and a single 10,000 Creon capsule with snacks. If I'm eating something contains a lot of fat, I take 2 capsules instead of 1. I thought it might be not enough creon for the amount you're eating that could be causing the malabsorption, but I guess not.
I just did a quick search online and the other 2 reasons why you might have malabsorption happening is either bacterial overgrowth, where bacteria in the small intestine is fermenting carbohydrates that aren't absorbed and leads to gas pain and bloating, or it's neuropathy that can cause diarrhea and steatorrhea. So it could be PEI which you're already taking CREON for and also a bacterial overgrowth and/or neuropathy.
I would suggest asking your diabetes doctor to check for a possible bacterial overgrowth, specifically in your small intestine (SIBO) and also get checked for neuropathy, specifically autonomic neuropathy.
