Hello, I'm Adrijan from Macedonia, 24 years old, was diagnosed with Diabetes Type 1 4 (four) years ago, and since then I'm injecting 14 + 10 daily units of Insulin (Novomix) Aspart 30.

In the beginning, everything was fine (except an annoying tingle in the legs afterwards spreading toward other parts of my body, which triggered on hot temperatures) and I was feeling fully energized, with only slight hypo's slipping here and there along the merry diabetic way.

But, two years since the diagnose, things started getting weird: first, I experienced severe fatigue out of nowhere (sometimes sugar blood levels were under 10 mmol/liter, other times they were above 20 mmol/l, but never above 30 mmol/l, and they were fluctuating in the range of 2 - 25 mmol/l. My HbAc1 for the time being was somewhere between 5,7 to 7% periodically in three months, but never above 7,1%, i.e. 8.0 mmol/l.

Then I went on the work and travel program trough one of our agencies, and was working in South Carolina, North America (changing time zones) for approx. three and a half months, a time which saw some changes in my energy levels and sleep patterns (usually, I slept very little and was experiencing severe amount of stress due to the long working hours and climate change). I was also feeling very tired most of the time, with the same blood sugar fluctuations, under which I took the liberty to inject multiple doses daily after severe hyperglycemia episodes because I was afraid of ketone acidosis or whatever horrors diabetic patients experience.

Then I got back in Macedonia (my country of origin) and started to have severe stomach or gastrointestinal pains, and one time I got so sick that I vomited and had a severe diarrhea for a week, which was preceded by irregular heartbeat, irregular sleeping patterns, general fatigue and stomach pains. Afterwards, I took some tests which showed that my heart was perfectly fine (yes, even a stress test) and I did a endoscopy (a tube in your stomach) after which I was diagnosed with a small hiatus hernia, and was prescribed some B vitamin, or Neurobion (after which I was getting very sleepy), Emanera (which is a beta blocker I think) and a digestive helper under the name Digeston.
Afterwards and some time before, I was experiencing stomach pain usually after eating a large and greasy meal (but not always), and the pain was starting in the abdomen and ending in my throat (or so I was feeling), which felt like it was about to implode on itself. The symptoms came and went, but were mostly present (4 from 7 days in the week, with intensity fluctuations). My energy level took a curve to the worse. In the meantime, right before I took the stomach test, there was a two week period when I wasn't sleeping at all, and it seemed like my heart would burst out in the fashion of a Xenomorph from the first Alien movie.

Afterwards, I did a blood test sample, which showed that everything is fine (iron good, urine negative, all crisp), except a slight increase in my bilirubine levels (35.95 mmol/l from the usual 24 threshold, and direct bilirubine level 9.06 from the usual 5.1 threshold). Also, before the blood sample test, I did a thyroid function test, which strangely enough, on the first try showed 3.29 TSH, 16.2 ft4 and 433.3 ATP-O, and the ATP-O went down in the normal range on the second test.

Today, 4 (four) years after the initial diagnose, I feel like my energy levels are way down from what they've used to be, and my blood control is tighter than ever, never going above 10 mmol/l or below 3 mmol/l; I also started cooking healthy meals and all, but sometimes I feel like after an insulin shot and a medium or large meal, my heart races against my body and I feel all shaky, or sleepy, and basically the next 4/5 hours I'm just battling with myself and trying to stay focused despite the irregular heartbeats and extreme fatigue/drowsiness/sleepiness. Sometimes I have trouble sleeping, but not always; also, sometimes I get tired after only one short trip to the store, when another time I'm making 100 push-ups (in 5 series) without a major problem. Usually I feel moderately tired upon waking, and severely tired after breakfast, with blood sugar levels in the normal range (before meal, 1 hour after meal, and so on and on).

And there you have it, my whole psycho-emotional life after the diagnose just poured into several paragraphs of boring text that's probably no one read. But if you do, did you had a similar experience, and most importantly, what can I do to make life better and feel more energetic and succeed in it (if possible)?

Hi,

I don't use insulin, and I'm a type 2 not type 1, so I can't comment on your insulin, I'm afraid.
But I would like to welcome you to the forum (and compliment you on your excellent English!)

Hopefully some insulin using type 1s will come along and be able to offer some experienced advice.

You may find you get more response tomorrow, or the day after (they are probably all sleeping off their Xmas dinners at the moment ;)) so please keep coming back to check.

And, if you have time, read as much of the forum as you can. This place is a treasure trove of information and advice.

Welcome to the forum. Has anyone suggested changing your insulin regime to one of basal and bolus? Also known as MDI? I can't say I recognise any of your symptoms in myself, so I wonder whether different insulin may be beneficial.

Hi and welcome to the forum Adrijan.

I don't know what health care system is like in Macedonia, but given your on-going health problems you should be seen by a specialist who deals in diabetes, if possible ask your Dr to refer you over to see an Endocrinologists and explain to them what you've said above. Good luck.

Hello and welcome to the forum, so sorry to hear of your ongoing problems. Have you been tested for Coeliac/Celiac disease yet. As I had symptoms similar to yours, especially the vomiting, I'm type 1 and type 1 and Coeliac are both auto immune conditions and are closely related. It may possibly be a digestive disorder ? or could be a unusual reaction to your Insulin ?? Also Thyroid condition ? or even just a change in yiur diet.

I do hope you can get the proper help and advice for your condition very soon, it's always best to go to your doctor or diabetes doctor, keep on at them to investigate and then eliminate, every avenue possible.

Coeliac sounds like a promising lead, Adrijan. Do you eat wheat/grain? If yes, try cutting them out?

Low carb seems to give more stable blood sugar control, but what you're describing sounds like a food intolerance, or perhaps a reaction to the insulin you're using ... Back to the endocrinologist for proper help, but it sounds as if you'll have to use trial and error to exclude certain foods or swap insulins.

Poor you. But be active and do something about it !!

Okay, so after all being said and done, the results were fine, except for some elevated ATP-O levels: 597.3 iU/ml (the normal being <34).

So, I guess I'm fine, for the time being.

Did you get a possible explanation for the raised ATP-O levels? (I'm sorry, but I have never heard of it!)

And how are you feeling now?

Hello, me again. Couple of weeks ago, I gave myself a permission to visit the psychologist's office, after which she concluded my introversion, melancholic type of person, and my various psycho-somatic anchors that fueled my anxiety since the initial diabetes diagnose.

Good news is those abdomen pains evaporated as they never were in the first place, and general blood tests show an overall positive results, except for some plummeted iron levels (on a 5 to 34 scale, it showed 8). I'm also doing recreational basketball plays: 3 weekdays with 1:30 hours per training session.

Bad news is, 4 to 5 days per week I feel mildly depressed and sometimes severely anxious over both banal and complex problems from financial, existential, health-wise and philosophical viewpoints. Also my sleep pattern is blown to hell as I cannot remember when I woke up, when my main activities should take place and what the hell represents a normal 24 hour biological functioning of a human body. That said, my sleep is poor, even if I take full 8 hours of it; also, the inhabited area around my apartment is full with noises (construction, discotheques, night clubs and whatnot) and that too is helping to disrupt my sleeping patterns (now full-blown chaotic and with a slight entropy dressings). So, as I'm writing this, daytime is sleep-time, and nights are for activities.

Another bad news is my elevated blood pressure, and chest and neck stiffness, which subsides after 3 milligrams of leksilium (muscle relaxing drug). Since my heart in fine (both from ECG and Stress test evaluations), I cannot pinpoint the direct (or indirect) cause for the aforementioned symptom.

Very bad news is that occasionally (more than rare) I get suicidal thoughts, and I'm constantly fighting within my inner self and battling with my psyche to prove the meaning of everything that surrounds us.

Again, all tests showed normal readings, so I'm guessing the brain plays games with myself?

Any help on the subject would be deeply appreciated,

Hey Adrijan, physically it sounds like you've come a long way, however mentally it sounds like you are worn down. It seems evident that depression and diabetes go hand in hand. I had a very low spell a year or so after diagnosis, my doctor put it down to my fluctuating blood levels as I was going high/low constatntly and I couldn't cope. I've adjusted my diet so I eat less carbs and am therefore more stable with my blood glucose.

I can imagine with how your feeling that you are willing to try anything, so if your willing to give this a go then it should help. Other methods I tried that work was meditating, so if you can find a group to join have a go at this and I hope you find some peace, good luck ;)

Hi - have you looked into gastroparesis as a possible problem? And looked into IBS?
