Hi

I take 56 units of Tresiba at night and 12 of novarapid with meals

There are many times when numbers are running really high even in the middle of the night they can be in 20s.

When this happens should I be taking more insulin at that moment to bring it down?

It sounds like you're on fixed doses of short acting before meals. Have you been shown how to count carbs by your nurse, and how to adjust your insulin? If not, give them a ring and ask for an urgent appointment, as blood sugars in the 20's are too high, and require treatment asap.
