We've seen a pattern of lows for Jess around 5pm each day. Her readings drop to around mid to high 3s. Today she came out of school at 3.15pm with a reading of 3.8 mmol (as a result of PE in the afternoon I think because she's not usually low at this time). We corrected with one glucose sweet which we know raises BG by around 2 to 3 mmol. After that she didn't eat anything and by 5pm was 3.0mmol. Previously we've done another fasting test and her reading dropped by 1 mmol during the afternoon, she wasn't hypo that day but didn't do PE that day either. I wasn't sure if it was the bolus for her afternoon snack causing the problem but as today she's eaten nothing apart from the glucose I feel it is the basal that needs adjusting. This is the second afternoon fasting test we've done and on both occasions she's had a drop in BG.
Currently she's been on a basal of 0.350 per hour over a 24 hour period. I think I've done this right but want to check. I've altered the basal from 3pm to 0.250 per hour, this runs until 6pm before it reverts back to the 0.350 units per hour. Am I right in thinking that if she's going low around 5pm then it's a couple hours before this that I need to step back the basal? Her ISF is around 1:5, i.e. 1 unit of insulin reduces her BG by 5 mmol. So I'm guessing that 0.100 units of insulin will affect her reading by around 0.5 mmol. If this is running for 2 hours then it might affect her reading by 1 mmol. I guess the best way is to test it and see.
On her Animas pump there are now three basals:
00:00 0.350 per hour
15:00 0.250 per hour
18:00 0.350 per hour
When the pump was set up by the rep they started the basal at 00:00. Should I leave the setting above like they are or should I or can I delete out the 00:00 setting. I guess it doesn't matter as it does the same thing but it seems silly to have the 00:00 setting in there now as it's the same as the 18:00 setting.
