Advice needed on testing before driving

No problem

So in efect, it's absolutely down to the drivers discretion in NZ regarding fitness to drive. With no intervention of a doctor's "reference" subject to the drivers medical condition managment to the licensing authorities?

Not absolutely, there are restrictions, but looking across the range of medical conditions involved, IMO they generally don't go far enough.

I'm not suggesting this applies to T1 though. In that area the restrictions seem similar to the UK and are probably ok:

I guess the transport authorities are more familiar with the ins and outs of T1 than they are with sleep apnoea. The latter has only really been treatable since the late 80s, and the number of people getting it has exploded with increasing obesity rates. It's still a relatively little known or understood condition.
 

Indeed. It would still be a "point of law" that any known medical condition (or its respective medication in the treatment of.) found to be a factor in the cause of a road traffic collision, would leave the driver open to prosecution..
