I am trying to fill out a ESA form
I am on ESA however need to fill out another form to explain illness but not sure what to write.
I have type 1 and arthiritis and neuropathy probs in hands knees and feet.
Gastroperisis in stomach
no energy always sick and tired
On medium care in DLA and low mobility
