Any help would be appreciated keto diet

I went for a pre-op for surgery last week however my HBA1C came back at 83 so they've postponed it until I can get it down I did keto last year for 4 months and got my HBA1C from 58 to 34 in 4 months however fell of the wagon and put the weight back on I'd lost and my HBA1C has risen, I've now started back on the keto diet and have been on it for 6 days now and lost 5kg which is good however my blood glucose is high despite eating minimal carbs any help would be appreciated,

Can you share what you've been eating just in case you're overlooking something?

Those numbers are pretty high. Can you test for ketones with urine strips or blood testing strips?
Is your age correct? 28 is quite young for T2, so I'm wondering if you've been tested for T1 after diagnosis.
 
Hi thanks for your response my age is correct a typical day for me is the following:

Tuna with low fat mayonnaise, pork shoulder steaks with scrambled eggs cooked in olive oil or butter and then I'd have a mix of 0 & 5% fage yoghurt with some protein powder from myprotein that has 1.3g of carbs per serving I'm roughly having 180g protein, 80g fat, 17g carbs a day.

I was pre diabetic when I was younger as I was very overweight and i had a glucose tolerance test done when I was 16 which confirmed I was insulin resistance.

I'm 6ft6in and weigh 181kg

History of HBA1C

2016 - 48mmol/l
2017- 43mmol/l
Feb 2019 - 65mmol/l
March 2019 - 58mmol/l
2021- 42mmol/l
Apr 2022- 32mmol/l (started keto in Feb 2022)
July 2023 - 83mmol/l

When I did keto last year my blood glucose was averaging between 4.4-7.7
 
6 days is a relatively short time if you’ve been running really high numbers, your liver has a lot of sugar stores still to dump, keep doing the keto and you should start to see a downward trend.

Another question is are you feeling ill, off colour or on any meds that aren’t for diabetes, if you have a low lying infection or are coming down with something this can definitely affect your blood sugars. Also how do you sleep? Not sleeping well is another one, make sure you are drinking plenty of fluids, preferably water, avoid sugar free drinks as some people react to them as if they are full sugar

Don’t give up - the weight loss is great and you’ll get there
 
Thanks for the response I do have non alcoholic fatty liver so not sure if that will also affect it, I feel perfectly fine only medication I'm on is trazodone to help with sleep and depression been on that for the last 10 years, and cosentyx(secukinumab) injection for psoriasis been on that for 6 years as for sleep I'm going to bed at around 3-4am and waking up at 11am, I do drink lots of fluid around 3-4.5 litres a day majority of this is diluted sugar free squash
 
Sounds like you're doing everything right to me. And what bad luck getting insulin resistance and T2 at such a young age, along with your other health issues.

I think the best thing you can do is see if those numbers want to come down with this, and if they don't to have an urgent talk with your HCP about medication.
Your hba1c likely means your average BG has been around 13, so lower than the tests you've shown here. Which is odd, considering what you are eating.
 
Try cutting the squash and drinking just water - hard I know, tell me about it! But you could be one of those that’s body is fooled that it’s getting full sugar. Try sparking or plain water - I get a big jug of water in the fridge and put some cucumber slices, fresh mint and leave over night it’s very refreshing either with still or sparkling or even council pop (tap water) a couple of strawberries cut up with mint makes a nice one too, lemon & lime. Obviously coffee and tea are fine either black or with cream. Green tea or iced tea, fruit teas or Twinings do cold fruit tea bags which are delicious- you just drop them in your water and let them steep for about 30 minute
 
I do have a diabetic review tomorrow so will be discussing the numbers with the nurse so will see what they suggest I'm pretty much eating the same as I did last year when I did keto and the numbers come down so hopefully with a bit more time I'll see some improvement

So update had diabetic review with healthcare assistant she's done my bloods so should have them back in a couple of days, diabetic foot check was normal no problems, showed her my readings and she's booked me an appointment on the 31st July with the diabetic nurse to look at going on medication so just wondering if anyone has and recommendations as I've been on metformin slow release before but stopped taking as I was worried about the lactic acidosis side effect

That's a side effect so rare that it's almost unheard of, except for people who are very ill (like close to dying ill) already. It has also not been proven that metformin had anything to do with cases of lactic acidosis, not even in people with a high alcohol intake.
 
Hi im in the same position. Waiting for an operation which was postponed because my hba1c was 97! I did keto 3 years ago for my sons wedding and lost 2st and hba1c came down to 54 all in 3 months. But this time around it’s taking longer and I’ve had to go on medication to help to lower blood glucose. Had my hba1c last week and it’s 62 so back on the waiting list. I’m almost carb free but still without medication my BG is around 9mmol/l.
just a thought about the diluted squash, only erythritol doesn’t spike my BG. All other sweeteners such as Xylitol and maltodextrin send my reading sky high So check the label on your squash as you might be the same

I was first diagnosed with non alcoholic fatty liver in 2016 via an ultrasound due to my weight didn't realise It was anything to be worried about carried on eating rubbish in 2021 I had quiet a depressive episode due to family deaths and started drinking alcohol 5 times a week for about 10 months gave up drinking in October 2021 and saw my doctor who ordered a ultrasound which found fatty liver and enlarged spleen so was referred to hospital who did a fibroscan which confirmed I had fatty liver with F2 fibrosis that was the end of 2021, spoke to the liver doctor and told him I was going to do a keto diet as I've seen it helps with nafld and he was more than happy for me to do it, I also started taking NAC, Vitamin e, Milk thistle and choline bitartrate after 4 months of doing this I had an ultrasound and was told my liver looks normal and my liver enzymes were back to normal I never had any symptoms of fatty liver

Do you think that your NAFLD will have returned during the time since you stopped doing keto - about a year or so ago?

I'm currently struggling my way through a Robert Lustig presentation (the audio is terrible) which discusses fatty liver disease. It's about an hour and a half long and won't be everyone's cup of tea but I'd be curious to know if there's anything in there that helps you.
