Yes, the HbA1c is a rough average of your levels over the previous 2 to 3 months so eating before the test will have no effect at all. However, if as @Squire Fulwood said you also had a cholesterol test you would need to fast if it includes triglycerides.
With an HbA1c of 52 there is no reason why you have to have medication if you don't want it. You could ask your doctor for a further 3 months on diet only and then another test. My HbA1c on diagnosis was 53. Medication was never mentioned.
