Insulin
Hi

7 units for dinner, If I go to 3.5 an hour after food, I eat 4 digestives (too many?) then go to mid teens, can I rehit to get that down? I know I shouldnt stack but if I wait 1.5 hours to avoid the stack then Im hitting no food, so I dont do anything and stay mid teens all evening and night until I wake and eat breakfast when I can dose

I hope that makes sense Thanks

Hi @Fenn,
For advice about your insulin doses you really need to contact your DSN or doctor.
When BSLs go low and then we eat to deal with the hypo then inject some more insulin it becomes
what I call the 'hypo-hyper see-saw' and it is difficult to get off it !!
Best Wishes

Fenn, I'm not going to comment on insulin stacking, as it's outside my experience, but could I ask you two things?

Firstly, are you on fixed doses of insulin, per meal? You talk about 7 units; is that the fixed dose for dinner, or your carb counted estimate of your need?

As I understand it, the ideal for treating hypos is to find something, like biscuit, jelly babies, quantity of sugary drink that raises your blood glucose by about 3, so that that can be a measurable "dose".

Of course it's then a case of eat/drink, wait 15-20 minutes, test then either "move along" or back up with some longer acting carbs to mop up any "spare" insulin still on board.

I wonder if your hypo treatment "dose" of 4 digestives is just too much in one hit?

Do you have a review coming up any time soon? Sounds like a topic for discussion.

Thanks for your replies

I am phobic about doctors/dsn, if they leave me alone I leave them alone, I have the tools to fix myself without help, I am just figuring out how to use them, I have it going well with the insulin, my nightime dose is fixed 19 units, but this was fixed by me finding a nice level all day and night without food.

I roughly know how much to take for the meal I eat, 7/8 is max, I had spaghetti bolognaise for that 7. The question was really if I can inject without food to adjust the high, say 2 units or if injecting without food is a golden rule, I did it once and that wasnt pleasant and dont want to repeat it but that was 6 units.

No low carb advice please as I mentioned the spaghetti

I agree with @kitedoc you should really contact your doc or dsn about your insulin use. It would seem to me if you are taking that much and an hr later you are 3.5 (Although you didn't provide a premeal bg level) then you took too much. Does this happen frequently?

I'm not a fan of doctors either, so I understand the phobia, but this is something you should be able to maybe get advice over the phone on.

Assuming correct diagnosis, as a type 2 you have insulin resistance, so your requirements for exogenous insulin are likely to be somewhat unpredictable over time, and likely ever more demanding in terms of quantity needed. I can’t help with dosing, and the only advice I would comfortably offer, you have already said you don’t want. Unfortunately you will probably find over time that this becomes an ever increasing problem, but good luck anyway.

Please understand @Fenn, we are not doctors or nurses. We cannot advise you on insulin doses and management.
Perhaps finding another DSN and doctor is part of what you need to do??

Thanks guys

I dont dislike my medics, in fact they are very nice, just dont feel I need them, its not rocket science

I was really just wondering what folks do when they fix their hypos but over shoot, not wanting specifics, just if people inject without eating, I totally get you cant give me specific dose advice, maybe I gave too much info that made it sounds like that.

No offence ment to lchf Jim Lahey, im a huge fan of lchf, I meant I didnt need advice, I have had very great success for many years doing it, i just dont need telling off for having spaghetti I chose what I eat, when I eat and accept the consequences, this is generally very low or reduced carbs but I do eat carbs when I fancy them

My type? Who cares, the treatment is the same, I cannot control my db with food and or tablets so the treatment is the same regardless of my type, as far as I know if `i were diagnosed type one this would mean more medical help and possibly courses, neither of which I want so its all good.

Thanks for replies

My type? Who cares, the treatment is the same...

The treatment is the same but as a type 2 you will be resistant to the insulin you are injecting. For the majority of type 1 this is not the case. That’s the only reason I mentioned the type of diabetes. I believe it’s relevant to your insulin response, but that’s just an opinion

I dose insulin without eating regularly, if I need to correct, then I correct. I do log each and every dose, so I’m always aware of exactly how much IOB I have.

Jim is right though - the type does have a relevance here. I’m type one and not metabolically compromised - I have healthy insulin sensitivity and only take small doses, I need it for organ failure. If you are type two and resistant to the stuff, things could be different re stacking.
