Endo Appointment tomorrow

I have an endo appointment tomorrow for a second opinion.
Yes, literally playing Russian Roulette here.

I have to eat every two hours, whilst having intense hunger symptoms,
sweating, faint like symptoms, and arrythmias.

The first endo I saw a few months ago refused to do a glucose challenge test,
on the basis that the one fasting blood serum was normal. Ignored the log I kept
of blood sugar level spiking up, the worse being 13.6.

Due to her intransigence and acceptance that one fasting blood sugar level,
comparing with the others I kept was normal, my heart failure has now worsened,
with a BNP of over 600 which carries a poor prognosis.

I will again take the chart to show a different endo at a different hospital,
obvious fasting glucose too high, and along
with very serious symptoms of diabetes, and worsening heart failure, neuropathic pains,
including horrid electric shocks in the body, I would need to have some type of management
put in place.

Question 1 - are there Nice Guidelines that needs to be followed in such cases? The point here is
I have to eat every two hours now, am in a state of collpase, can no longer stand up.

Question 2 - any suggestions on how to put my case across?

Question 3 - since I am now very very frail, and already suffered Acute Coronary Syndrome,
which I believe was caused by the undiagnosed diabetes, and the worsening heart failure, also
caused by undiagnosed diabetes, is there a test that is perhaps not as brutal as the glucose challenge
test, but a gentler method instead?

I am in a constant state of confusion, feeling light headed and collapse. Its no use going to A&E,
they will not offer the test, other perhaps that a glucose serum test and refer, or unless in a coma, not tests
will be carried today or tomorrow, that much is obvious.

I live in London UK, and will be going to Chelsea and Westminster Hospital, Fulham.
