Likewise, even though have tried fasting before bed or eating some carbohydrate before bed (with and without metformin) I can't get rid of the dawn phenomenon readings of 6.1 - 6.5.

You have nothing to eat between getting up at 7am, and testing at 4pm?
If that's the case, you should do a search on liver dump or dawn phenomenon. Our livers can gives us a dose of glucose from our own stores if we fast a very long time, or get very hungry. I'd say that's what is happening.
Try testing as soon as you get up, and see what that shows.
