HbBA1C jumped from 42 to 69. Not responding to low carb diet.

My partner has managed his t2 with Met and diet and exercise for 7 years. His hbA1c bobbed along between 42-46. Suddenly in November it jumped to 69.
Since November I have cooked low carb meals ( around 40 g carbs per day) and we cannot get a grip on his BG. He got fed up yesterday and ate a roll ( a single white roll) and BG was at 14. His Met was doubled to max dose in Nov. It appears to be have no/little effect.
Any advice? He is becoming discouraged but won’t go back to his DSN until March.
 
How his bg respond to fasting? Have you tried low carb and low fat? Is he overweight?
 
Can you tell us what a typical days menu is? Is he testing before and after eating? Is he on any other medication,?
 
He fasts most days until 4pm, OMAD. Salads, chicken and cruciferious veg/mushrooms/ bone broth/ omelettes/ these kinds of things. I am keto so he tends to have what I have. he is at least 2 stone overweight but low carb isn’t helping there either oddly.
 
He isn’t eating low fat. I don’t believe low carb and low fat go together- otherwise you eat too much protein and you nights as well eat carbs.
 
Have you considered reducing his carbs to 20g per day?
 
Yes, I will try that but he sees that as keto and “a step too far”. He says it’s not sustainable but as I see neither is his life at this rate.
 
Has anything happened in November? If we have an infection, stress, statins will all raise bg.
 
He quit statins a few months ago. I suspect there might be an additional health condition but he refuses to seek any help. I hadn’t directly correlated it to the spike however I now see that’s almost certainly what it is. Thank you for the clarity.
 
Most of us prefer not to have statins unless we have had a cardiac event. I know of people who stopped statins because of the side effects. It took a while for the side effects to subside.

Low fat and low carb equals starvation. We do need fuel to live.

What is his BG like when eating low carb?
What is he eating over a day? Can it be it's higher carb than you think?

His blood glucose is between 8.5-12 low carb. I often wonder what he eats when I am not about. However if that’s what’s happening I am quitting with the low carb food prep for him. I invest a lot of time and money in his diet.

I think his other illness is connected to statins so I am glad he is off them, although I now wonder if he is headed for a cardiac event...
 
Is he testing before and after eating, aiming for a rise of no more than 2.

It is possible that he is eating stuff out of your sight. You need to have a Frank discussion. Can he take a lunchbox to work with something like, cheese, olives, nuts, pork scratchings etc.

Can you attend his gp appointments with him? Especially if you think there are other conditions.
 
He is retired. There is no way I can attend appointments with him. Not a chance. Have tried the frank chat on numerous occasions no of which ended well. We are at the end of the line unless there is some personal responsibility.

I went behind his back and spoke to his gp. She couldn’t have been any less interested.
 
You are right, it is down to personal responsibility. He may just be fed up. Some nice keto desserts may perk him up.

I don't think a GP is allowed to discuss a patient with someone else without their consent.
