I see your post has been unanswered, maybe missed so bumping you up again.
I know you say your unable to get support from your diabetes clinic, but sounds like a few days with a continuous glucose monitor on loan from your hospital may help with your basal testing, enough maybe for you to tweak it a little to help you get them right.
How often do you test a day normally? i know your neuropathy can cause you more pain by testing more but it may seem the only other way is to stick at it with a basal fasting test. It may be more worthwhile in the end as more stable BG's will maybe ease some of your tiredness.
You could also do some research into alternative site testing so you could spread out your BG's tests along more areas to help with any discomfort.
Sorry i can't be of more help, the only few suggestions i can think of, maybe none will help but just thought i'd try, hopefully someone with more experience in your situation can offer better advice.
