I have Just done all this and I also went down the wrong path filling out the DLA form myself :cry:
PLEASE DONT you may well get turned down as I did
eg
I am stuck in a wheelchair due to a accident I was refused DLA 1st time around
The refusal letter came back saying that I could walk over 400 meters ? :shock:
I had to go to court to overturn this with consultants letter saying that it would be imposable
Get help from CAB

CAB will help you and fill forms out for you CAB will also help if your turned down
or you may have local welfare rights officer who will also help
Include as much proof as you can
Include letters from your doctor
Include letters from consultants
Include anything that will strengthen your case
