Libre 2 sensor

Hi all, I am moving areas from London to up North and was wondering how I sort out my transfer of my Libre 2 as the NHS hospital im at now was the one that sponsored me to get it in the first place, would it just transfer to my new hospital and then I have to share my information with them ?
