'detailed coded record' seems anything but 'detailed' or 'coded'. I suspect incorrect coding.
Is it possible to see the SNOMED codes? Would a Subject Access Request show them?
I can't find any examples.
 
It’s unlikely even a SAR would provide the codes I mean you could always try but if they don’t you’ve wasted time requesting and waiting for an SAR to not be provided them, you’d probably have to specifically request them. Whether they’d actually provide them though is another matter
