Whats the proper name for the small machine we use to make our finger bleed? Mine's is a bit unstable just now. How do I get a new one - whose responsibility is it to provide new one, or do I go to Boots and buy one?

It's a lancet, what's unstable about it. The little machine to hold the lancets is usually pretty robust. You do usually replace the lancets only. It depends do you get you strips etc from you GP? If not then you will have to buy a new one. Don't think they are dear tho'.

Yep they're lancets.
I would suggest you contact your meter/lancet supplier as they usually send out a "pricker" to keep you using their product (just like they do with the meters )
