Type 2 Need your help on designing a Clinician-based Wellness App

We are designing an app to help diabetics manage their lifestyle goals. These goals will be prescribed by registered health professionals and automatically loaded to the app. We are thinking we will include the following categories: exercise, nutrition sleep, stress, general diabetes management, and provide a localized social forum.
We are looking for tips and want to hear what you would find useful.

Why do people persist in believing that apps like this will make a difference?

There are two types of people, motivated and unmotivated. Getting hcps to set goals doesn't motivate people.
 
I kind of agree with Tim. I guess it would be helpful to know though exactly what these goals on an app would do to help? Or will it just be more pressure for us?
 
We are hoping this is a is a way for clinicians to upload simple and and achievable goals . We are looking for tips to make it was useful as possible
 
So it's goals from the doctors for us and they are uploaded so the doctor next time can check but we do nothing is that right?
 
Its probably no different that what happens now (ie. the doctor/nurse agreeing with you what you might want to work on) but the difference is making it easily available to you (rather than a piece of paper or conversation). And providing access to community support as well.
 
Okay, can you outline your project and what you are trying to achieve and allow people to provide feedback? That may help with determining what you develop and how it operates?
 
Ah I see. To be honest I do t think it would help me. I feel the after report and notes I take are enough. I can see I. Ways how it might help some but I'd guess most wouldn't want the constant reminder
