Patient-Centred Care Plan

Thanks for your personal experience of surgeries, @DavidGrahamJones. That makes me wonder if I'd get the six-month one but as I was told by an admin staff that no further action was needed, I thought I'd ask for the three-month just for my own peace of mind, but that might be superseded as the six months is coming up. Fingers crossed!
 
My GP's been pretty good I think. I've had quick referral for retina scanning and my foot check last week. The GP's has a retina scanning facility on site, and podiatrists did my feet. I was probably referred to podiatry due to having limited circulation in one leg (blocked stents), so a higher risk. Podiatry rated me moderate though, and wants to see me every 3-4 months with a basic care/checklist on my booklet. And I've got my HbA1C and review scheduled in. Kinda looking forward to that and hoping the low carb advice from here has paid off.
