Hello

Just a quick question, I am on a Low carb diet and low calorie typical diet :

Breakfast : 3 tblsp Low Fat Greek Style Yogurt & 1.2 Grapefruit (12g carbs)
Lunch : Either same as breakfast (12g) or 2 x Ham Cheese Lettuce sandwich (no bread, Lettuce used instead) (0g carbs)
Tea : Chicken of fish salad with Greek Yogurt afterwards (max 15g carbs)

I have to key 15g carbs into my pump for breakfast, 20g for lunch and 25g for tea to ensure BG levels stay within 5mmol to 12mmol.

My carb ratio is 1unit to 2grams which is very high in comparison to others in my insulin pump group (there was only one other girl on similar daily insulin requirements to me but she was over 5 months pregnant!).

Why do I have to use so much insulin for such a low carb diet and small protein portion. Does anyone have similar issues? I'm just under 11st and want to get to 9.5st, but weight isn't budging. I do alot of walking each day and try to keep it low intensity due to keto effect. Someone in my insulin pump group recommended I ask to go on metformin as he had similar issues and called it double diabetes?? Metformin really helped him to bring insulin requirements down and weight. Any ideas greatly appreciated.

I have questions (hopefully they might point somewhere helpful):

  1. Are you confident in your carb counts? Half of one grapefruit could end up having a very different carb content to half of another grapefruit - do you weigh your portions? Do you check the carb content of your ham & cheese? And I think lettuce has some carbs in (Google says 2.9/100) are you sure your lunch is 0g carbs?
  2. Are you confident in your basal settings? Have you done basal tests recently?
Double diabetes is when you are type 1 and also insulin resitstant, which is kind of type 2, it might be worth looking into metformin to help with insulin resistance.
