what am I doing wrong?

Hello, I hope somebody can help me, I'm type 2, on insulin, I take 12, 14 and 14 units of novorapid and 46 units of Lantus a day I also have 2000mg of metformin and 4mg of Glimeperide, but I can't get my bs down, my fasting levels are around 20 and after meals is around 23mmol. I've cut back on the amount of carbohydrates I consume, although I don't carb count, I don't eat or drink anything sugary at all. The only thing I can think of is my liver, I have had raised enzymes and was wondering if that could be a contributary factor, has anyone ever heard of this?
I'm due to see the endocrinologist next week and I was hoping to have normal levels by then, can anyone offer any advice please? My last HBA1c was 10.6%, I'm really fed up now, I just want to get to a normal level and feel better.
 
How abou keeping a food diary for a few days and seeing if you are actually eating more carbs than you think you are?
