I declared my first fit note to UC on 23rd Feb.(not that a fit note means anything as I still have to attend MANDATORY appointments with my Work Coach and with the Restart Scheme.)
I was then told by my Work Coach I would start a health journey and that I would be sent a questionnaire and then a work capability assessment will be arranged to assess my capabilities to work.
20/03/24 I got a message in my UC journal saying you have been referred for a Work Capability Assessment.
You will receive a questionnaire in the post. Please keep submitting continuous fit notes to keep on the health journey.
You will need to keep attending your mandatory appointments with your work coach to keep your entitlement to Universal Credit. I received the form in the post. Filled in said form and sent it back using the envelope they provided!
I messaged them on 28th May asking for another form to be sent out to me as I was told by my Work Coach that they hadn’t received it! This is the message I received back from them!

You health assessment referral was closed by the Centre for Health and Disability after they didn't receive your health questionnaire back within the required timescale. That is with a Decision Maker at the moment to consider whether the health journey should continue which is the process and the Job Centre have noted your claim to say when you posted that back. If it is decided that that should re-start a further form will be sent to you. (FULL OF ****!!!)
The deadline for the form to be sent back by was 22nd April and was sent back in plenty of time!!
