My student is writing an eesay on the links between age, length of time since diagnosis and complications. Would anyone be willing to complete this short, anonymous questionnaire on Google docs?

My student needs to research the relationship between length of time since diagnosis, age and complications. Could you please tell me how old you are, how many years you have been type 1 and if you suffer from any of the following?
Diabetic Neuropathy (nerve disease)
Diabetic Retinopathy (eye disease)
Diabetic Nephropathy (kidney disease)
Cardiovascular disease
Fatigue
Problems with healing wounds
Blurry vision
Skin problems, such as bacterial and fungal infections
Loss of weight
Frequent headaches
Teeth and gum infections
Bone and joint problems
Feet problems caused by damaged nerves or poor blood flow
Finger and toe numbness or reduced sensitivity

Happy to help but there is no option for <5 years since diagnosis.

Likewise - 0 years for me.
Newly diagnosed people mightn't seem relevant, but, for example - some people would've had some of these issues before they were diabetic - which has to figure into any data analysis done

I filled in the questionnaire for you but found that if you don't suffer any complications ther is not a "none of the above" option
