Hello everyone
hope u all in good health
this week i'm going to famous specialist in DM and i want to change my regimen to basal bolus type as alot of people told me it's more convenient to DM type 1 patients for me i have been focusing on lowering my Hba1c moved from 8.5 to 7.4 in last quarter
by focusing on reducing carb amount. i want to make more and more control so i can reach below 6.4 of Hba1c. for me i'm using mixtard insulin" which is a mixture from fast and intermediate insulin" 30 iu in day and 20 at night with actos pills "pioglitazone" 30 mg apill per day. and neurobion tablets as source for vitamin B12. last week i have very disturbed views to my blood sugar that some times reached to 400 mg/dl after 2 hours from meals and the fasting reads were 120-150 mg/dl. i need to take about 30 iu as a third injection so i can reduce my blood sugar it's like the insulin vial was corrupt or not 100% effective. Anyway i would ask about somethings
is it wrong to take a corrective dose by myself ?
is actos pills has any role in my condition "as i'm type 1 patient"?
i couldn't examine any hypoglycemia even if i didn't eat anything is this mean my body become less sensitive to insulin ?
any other thing i could remember i would add it to the comments

Basal/bolus should help you :) The book Think Like A Pancreas is very useful.

To answer your questions:

- only correct if you know what you're doing. You should be given a correction ratio so you know how much insulin to give.

- I believe Actos is mainly for Type 2s. Why not ask your doctor why you've been prescribed it?

If you feel your insulin isn't working like normal, always start a new bottle/cartridge.

Let us know how you get on with basal/bolus :) Make sure you're shown how to carb count and adjust your insulin for meals.

Hi. In the UK all T1s are recommended to have Basal/Bolus; there really is no point in staying with mixed insulin. It should enable you to have better control. The Glitazones are now deprecated as a T2 medication. Although they can work quite well, they can have some nasty side effects. Having them with insulin is very unusual. Corrective doses of insulin can be taken with Bolus but again unusual with mixed insulins. In summary do consider Basal/Bolus and I would expect you to find it much better for you.

I went to my doctor and he give me basal bolus regimn bolus was apidra 7 iu before meals and basal tresiba 20 iu before bed he said i should stop taking pills and use neurobion tabs as a prophylaxis twice daily as they are penfill i read that i shouldn't refregirate them and keep them in room temp below 30 Celsius should i do this for both penfill or just the tresiba one and should i shake them before use or not thank you ?? I should monitor my results and tell you what is going on ?? Thanks
Another thing should i shake the penfills before use thank you ?

I have been a week using basal bolus but with no real effect it's getting worse always high in fasting and post predial reads should i return to mixed insulin i used or should i increase doses for example im 320 after meal how many units should i use to reach lower than 200 mg/dl pls help i have another appointment next week but all reads are bad all of them thank you
