    I started getting this error when moved to the libre 2+, didn;t happen on the old libre 2 sensors. Have since got a new phone and installed the app from the playstore and issue persists. Thinking it was an android issue i added the app to my work iphone and guess what? Issue remains there.

    Took wrong insulin

    I did this before, but I was on actrapid at the time. Took 40u of Actrapid instead of 40u Insulatard, as we all know Actrapid has a slow release profile over 8hrs, so wasn't really an issue. Just waited till morning to take Insulatard after eating some carbs to compensate.
