Because (as far as they're concerned) I'm effectively using it "off-label". All the official docs say "Once, possibly twice per day." And there, again, is the problem. Twice a day for something with a duration of action of less than 5.6 hours? What am I supposed to do for the other 12.8 hours...

Because they think that levemir / detemir has an invariable (non-dose-dependent) duration of 24 hours.
