Xultophy Update May 2026.
Still low carbing around 75/80 daily. Have maintained the 12kilo weight loss.
Last 4 A1c’s have maintained at 38 or 39.
Saw an endocrinologist in December…two plus years after starting the Xultophy. I’ve been self funding the Dexcom1+ and maintained my time in range above 95% over the last two years… he says I can’t do much better than that which was nice of him. But he didn’t offer to supply it through the NHS!
I’ve tried to come off the insulin several times but with a corresponding increase in my glucose levels despite diet /eating pattern unchanged. He arranged for a C peptide test which shows I’m making some insulin (so not type 1) but insufficient for my needs but happily no longer insulin resistant.
I take around 16 units a day which seems to keep me ticking over. I don’t suffer hypos although I can drop to 4-4.5 and feel hungry, mostly before dinner time.
I feel good! I’ve come off the statin, omeprazole, paracetamol, metformin and amlopidine. I take one medication for tachycardia and a handful of vitamins. I use castor oil rubs to help with arthritis in my knees and fingers but that also improved with the low carbing.
I no longer suffer from excema and itchiness. I’ve reduced the use of my inhalers too.
I’m 77, active …walking my dog and using an exercise bike plus gardening when weather permits.

I’m Type 2 and it is very rare for a Type 2 to have DKA, but the hospital told me it was because of my SGLT-2 type medication (dapagliflozin) and an underlying infection. My treatment involved Insulin IV amongst other things and when I was stable, they took me of the dapagliflozin and moved me on to insulin injections. The main problem I am having with the insulin is weight gain and the inability to lose weight, and on investigation discovered that insulin prevents fat burning. I have been “ketoing” for 40 years now, on and off (Atkins since the 80’s), always successful when on.
I have been 100% on keto since my DKA, taking between 0 and 20 grams of carb a day – most days at the lower end of that, but can’t even reach nutritional ketosis. I explained all of this to the DN, and they mentioned that Xultophy may be something they would consider trying me with. I’ve not really been able to find a lot about it online (which is ringing alarm bells). Even investigating treatments for T2, it’s never mentioned. The only thing I have found is the basic information and a couple of case studies so it’s great to hear from someone who is actually using it with success. The case studies both mention weight gain but I can see this is not the case with you!
Like you, I self-fund my monitor which is Libre. NHS is so short sighted – I think the monitors are key in helping us keep control of our diabetes, think of all of the £££ they could save down the line when there are less patients with Diabetes complications!
As an aside, I’ve recently found out that the Nice guidelines for SGLT-2 medication inform that I would be at greater risk of DKA if I was following a very low or keto diet while on this drug. Also, I should have been asked to come off that diet before starting the medication. This never happened and I was put on the medication without any consultation. Rather angry at that but am much more informed all round now due to my investigations and through reading the feeds on this site.
