50% to much basal kind of urgent issue

Hello All insulin users,

We have just injected our 5 yr old child with 50% to much basal
7.5 units levermir rather than 4.5 (this is the dose for 24hr)

Anyone give me a clue on how much additional food I am going to need to give her
over the next 24hrs ?

Will it vary (so just react to the meter) or could i calculate a rough amount
(all novarapid is at a 12g carb to 1 unit ratio)

Would hourly testing in the night be OK

Hi, Sorry to hear that Matt. We made a mistake last week by giving our daughter levemir instead of her novorapid with her breakfast. We did stay up all night testing each hour and yes, I think the snack ratio sounds like a good starting point.

out of interest did BG drop much during the 24hrs ? did many hypo's arrive ?

Hi Matt, the insulins strengths are supposed to be the same so 1 unit of NR should be the same as levemir but over a longer period. Having said that you don't want to give her a load of carb all in one go. We managed to get good control but did have to test through the night and give snacks (fruit juice twice). Our daughter is 7 and we normally find that 5g of carb will raise her bg by 3 mmol. We were giving her enough carb to take her up to around 12 bg and then checking every 2 hours through the night. But as you've just given your daughter it now I think you will need to check every hour. Probably easiest to have her in your bedroom and set your alarm which is what we did. At least it is the weekend tomorrow so you can all have a sleep in then!
 
I can't help but wanted to wish you and your daughter well. I get in enough of a fluster giving my child occasional antibiotics. It must be so hard to keep a track of your childs meds.
 
Just wanted to let you know that most (maybe all) of us have done this sort of thing at least once. I actually gave my son his basal dose as his bolus and had to give him huge amounts of carb in a very short amount of time. :oops: I suppose it was lucky I noticed. Since that event I have a better routine for how I handle insulin and have marked the Novorapid pen (both are exactly the same) with stickers of rockets. :D

still only 12.30am here so a while to go
if 1novarapid unit = 1 unit levermir but just spread over longer period
then novarapid is 4 hrs and levermir is 24 hrs
so levermir is one sixth as potent
then if i gave 7 levermir units this about 1.2 equivalent of novarapid per hour
as we would normally give the equivalent of 0.8 per hour
we are only around 0.5 novarapid more than needed per hour
this would be around 6g carb
so the 24g of cereal given at 10pm should hopefully last 4 hrs
so a snack at 2am and then 6am might see us through
fingers crossed.
 
You haven't overdosed too severely and at least you did it with the long acting and not the short. Keep testing and you'll all be fine - except for being very tired. I often find when I increase Khaleb's Levemir that it lasts a lot longer rather than just bringing the bsl's down over the same amount of time. You'll laugh about this night eventually.

I guess you just need to keep testing and keep the lucozade handy. Could be worse - I gave myself 30 units of quick acting right before bed instead of my lantus. I mostly saw that as a good excuse to eat a ton of biscuits! Now I use syringes for my QA (I hate the pens) and a pen for my background so I can't mix them up again. Oops.

I know I've come in on this a bit late but I've always been told that if you alter your dose of long acting it always takes about 3 days or so for it to have any effect. In other words, if you have accidentally overdosed then hopefully the effect won't be too drastic as long as you don't make a habit of it and do it for 3 days on the trot.

I may be wrong tho, and in any case it won't make any difference to you now. Can anyone shed any light?
 
I haven't done it more than once at a time but can say that once the 24 hours for Levemir or 4-5 hours for Novorapid are over there isn't really much cause for concern. Changing Khaleb's Levemir dose permanently does take a few days to see the full effects.
