Pick up the phone and ring the surgery. They will sort you out. My surgery never gives me a blood form. It is computerised. I make an appointment to have my blood drawn and when I turn up the health care assistant has the form ready printed out. You need to be quick though!

I get given an envelope for my bloods from the hospital, but sometimes it doesn't survive the 6 months between visits. If that happens I just book a "diabetic bloods" appointment with the nurse at my local surgery (GP) - the results are visible to both GP and hospital.
