A written care plan???

soaps on telly and people talking about the characters as if they were real.
I spent a long time as a teacher and in the last years, there was a proliferation of paperwork. Most of it was just a pain, but one idea, I liked
All childen with statements of special needs had an Individuale education plan. (IEP) a copy of it was available to every teacher who taught that child, so we knew the specific targets, for that child.
Why doesnt the NHS have a scheme for written care plans for long term patients in outpatient care. I believe the in-patients sometimes have them.
A plan agreed by patient and care team, would lay everything out clearly. It might even make a dent in the Postcode lottery
 
In-patients all have them,Hana.I hate writing care plans,but then that's just me!!!While it is a good idea in theory I cannot see that the already overstretched PN's would have time to write individual C/P's for every diabetic on their list.However they do have our details there on the computer.Some sort of small statement on how we would like our treatment to run would be good so that you don't have to repeat the details to every new PN that you see.
