Am I really a type 2?

Hi,

I was diagnosed in June and was started on 80mg x glicazide a day.

6 Months down the line and im now on 160mg glicazide twice a day, plus 500mg metformin twice a day. My fasting average is 9.9.

I spoke to the dns at my doctors clinic and she said we need to consider insulin.

Could somebody advise what to expect?
 
Hi. Has your GP done an Hba1C test which should be done at least annually and more often until stable? Perhaps the 9.9 you indicate is in fact an Hba1C result? If so it is very high.This blood test shows your average BS over 3 months and most GPs rely on this to indicate whether any action is needed. If it is an Hba1C result then I can understand why your doctor is thinking about insulin. Anything above around 7 implies more medication may be needed assuming you are having a sensible low carb diet etc. When my Hba1c crept above 7 my doctor and I agreed to add Sitagliptin as the 3rd and final tablet before insulin. It has helped reduce my spikes. My GP is very anti insulin so was looking for another tablet to avoid insulin! Other GPs are more accepting of the inevitable. If you are in your 20s/30s and not overweight then it's possible you have a Type 1 variant and not Type 2 but the medication routes are similar regardless; your doctor can do extra tests to determine if a Type 1.

Thanks for the reply.

When i was diagnosed my fasting glucose was 14 and my hba1c was ll%. I had it done in aug and it was 7% and then again in mid december and was 8.3%. From diagnosis to now, each time i take a pill it works for a while, than it starts to creep up again.

My only weight problem is i have a slight tummy. Im certainly not fat/obese. I will admit im not as fit as i could be.

My diet isnt a bad one. I dont eat between meals, i dont have any sugary drinks, i drink decaf everything.

My weight did spike and go up a stone which i was told was due to the glicazide, however i have lost almost all of that weight.

I am 33 years old, BMI is 28, waist is 36"

I was much the same when diagnosed, started on metformin and a health diet, and for a while it worked, I was dismayed when after my first good HBa1c which was seven had crept up to 8% and I started taking glicazide which really didnt do very much. I was on the same dose as you.

In november my readings had gone up and up and hit the teens and 20s and my doc said insulin was needed, That was a month ago, I started on a once a day injection of Levemir a long lasting insulin,
and my readings are steadily coming down with the help of a lowish carb diet and exercise.

I have felt such a lot better on insulin, my energy levels have soared,

They cant make up their mind if im 1.5 very late onset or type two. Im having the peptide test soon

Im sure they will help you get to the bottom of your high readings and Im wishing you all the best.

HI. It does look suspicious that you may be a Type 1 variant. The C-Peptide test should show if your insulin production is really low. Although you avoid sugary drinks do you also keep all the carbs down as they can have quite a large effect on BS levels.
 
Hi All,

After a chat with the dsn she has agreed that its turning out to be a bit of an uphill battle, so Im starting insulin next monday.

I guess I am very lucky as I am cared for by by doctors d team, whos nurses split their time between the surgery and the hospital diabetes clinic.

She has talked about the peptide test but said that we should get this under control first, as the result wouldnt make the care any different.

Im really looking forward to this alleged energy boost. I know the pills make me feel slightly grotty, but ive forgotten what it feels like to not taking pills
