Any tips on insulin doses with high fat meals?

i tend to stick with low fat meals day in day out at Uni but as its birthday week I've been out tonight for a meal and getting takeaway tomorrow with flat mates etc.

Problem is the reason why I stick away from high fat meals is the late spike making my blood look fine for first few goes but spike up massively later on (fat takes longer to digest etc etc)

What I'm interested in is what sort of advice people can give for insulin doses in and around this?

Funnily enough, it's keeping me up now and I don't enjoy it. Annoys me heavily
 
@nskelton, without a pump there isn't a vast amount you can do. What you are trying to achieve on MDI is something similar to the multi- or dual- wave bolus, where you are trying to provide a spike of short acting bolus with the meal to handle the initial carbs, then with the multi-wave, you deliver insulin in an extended fashion over a number of hours to stop the glucose levels rising.

Personally I've found that that period, depending on food, can be as long as six to eight hours, thanks to the Libre. When handling this on MDI, I usually give short acting shot around the meal (depending on the meal, I may also have a delay) then further rapid acting at roughly 2, then 4 hours afterwards. I've found that I need about 50% of the initial dose in each of these shots, but the only way to determine what you really need is to test your way through it.

On MDI I would have to split-dose for high-fat meals to deal with the late spike, depending on the meal it would sometimes mean 2-3 injections as fat can effect bg levels for several hours after eating. On a pump you can use the Extended Bolus options such as the Dual-Wave Bolus.

The key is to test your bg regularly and learn from the results, keep a diary for such meals for future reference.
