blood sugar / glucose log book

Hi

Does anyone know where I can get a blood sugar log book from, I know you can download some off the net but I want a book to keep it altogether I know I could probably get one from my hospital but (**** excuse, but) I haven't got time to go all the way up there.

If you use insulin from Novo Nordisk i.e levimer, Novo rapid etc, you can ring them on 0845 600 5055 or visit http://www.novonordisk.co.uk and request a monitering book, they send them free to users of their insulins.
