I have befriended an older lady (69) diagnosed with type 2 diabetes 11 yrs ago, Metformin and Gliclazide not working for her, she was put on Insulatard injections plus reduced carb diet. She is not very mobile due to neuropathy in both feet. Since starting insulin she finds that she has needed to increase the dose, sometimes she injects and it has no effect at all, is that possible?
I haver her staying in my house at the moment as her home is undergoing alterations and she didnt want to go into an old peoples home, this has given me a chance to confirm that she is most definitely not eating anything in secret, she has also injected her insulin in my presence.
On Monday morning she injected her usual 40 units of insulin, fasting bg was 5.4 (she told me that is only due to her being naughty and disobeying Drs orders by injecting twice a day), I counted carbs very carefully, she had 45carbs during the day, yet her bg leveshe ls before bed were 16.3. She was told to inject only once a day as this type of insulin is long lasting. She wanted o inject again to bring this down overnight, I hardly slept a wink listeing out for her having a hypo!
Tuesday morning, fasting 5.7 (she had 80 units of insulin the day before), she injected 40 units as usual, she said she probably needs to inject even more to get the levels that she wants, I suggested try 40 carbs again, surely this will work, she said she wants to do an extreme low carb day, she had 2 slices of smoked turkey breast and a bit of cucumber for breakfast, 2 hardboiled eggs and celery sticks for lunch, 5 tablespoons of full fat Greek yoghurt with sweetener and sprinkle of cinnamon, no tea, no coffee, only water to drink, her bg last night was 14.7.
Is it possible that insulin doesnt work? She is due to see her consultant at the end of February. She now says that she is going to increase her morning insulin and continue injecting twice a day.
Surely there is something not quite right there, so much insulin for so little food, its not normal or is it? She has tried to inject in her stomach, arms and legs to see if insulin will work better, but it has not made any difference. I am losing sleep, I keep having thoughts that there must be vast stores of insulin in her body and what if it suddenly decides to release the lot or something, real nightmare scenarios in my mind.
I dont know all that much about insulin, can anyone help us out please?
Many thanks in advance
Judith
